摘要
颈椎前路减压植骨术是当今颈椎手术的主流,在颈椎外科领域取得极大成功。但是,颈椎前路减压植骨内固定术的缺陷被认为是,影响了颈椎关节活动度并引发相邻节段退变。在此理论基础上,颈椎非融合技术产生,颈椎人工椎间盘的相关研究取得一系列进展。现针对颈椎相邻节段退变、颈椎人工椎间盘的进展进行阐述。文章分别阐述了颈椎融合后相邻节段退变、人工颈椎间盘的历史和应用现状、人工颈椎间盘的设计和人工颈椎间盘手术相关事项。总的来说,颈椎椎间盘假体置换是一种旨在保留节段运动的手术方式,是颈椎融合手术的一种可选的替代手术方式。
Anterior cervical decompression and fusion (ACDF) is the main stream of cervical surgery,and achieved great success in the feld of cervical surgery.However, ACDF is believed to cause adjacent segment degeneration. On the basis of this theory, a series of advances in the study of cervical total disc replacement have been made. The progress of the adjacent segment degeneration and cervical total disc replacement are expounded.This article expounds the adjacent segment degeneration, the history and application of cervical total disc replacement,the design of the artifcial cervical disc and the related issues of cervical disc replacement. In general, cervical total disc replacement is a surgical procedure aimed to maintain segmental motion and is an alternative to cervical fusion surgery.
