摘要
目的:探讨湛江中心人民医院(以下简称"我院")住院患者抗菌药物使用情况与鲍曼不动杆菌耐药性变化的相关性,为临床合理用药、控制感染提供参考。方法:回顾性统计2010—2016年我院住院患者抗菌药物的使用情况及同期检出的鲍曼不动杆菌对常用抗菌药物的耐药率,采用SPSS 17.0统计学软件对数据进行相关性分析。结果:我院鲍曼不动杆菌对哌拉西林他唑巴坦、头孢哌酮舒巴坦、阿米卡星、美罗培南和亚胺培南的耐药率与亚胺培南、头孢哌酮舒巴坦的用药频度呈明显正相关(P<0.05);鲍曼不动杆菌对阿米卡星的耐药率与各抗菌药物的用药频度呈明显负相关(P<0.05);鲍曼不动杆菌检出率与哌拉西林他唑巴坦的用药频度呈明显正相关(r=0.828,P=0.021);耐多药鲍曼不动杆菌检出率与头孢哌酮舒巴坦、亚胺培南和特殊使用级抗菌药物的用药频度呈明显正相关(r=0.850,P=0.015;r=0.868,P=0.011;r=0.895,P=0.006),与阿米卡星的用药频度呈明显负相关(r=-0.845,P=0.017)。结论:我院鲍曼不动杆菌对常用抗菌药物的耐药率普遍较高,鲍曼不动杆菌耐药率变化与抗菌药物的用药频度有相关性,应加强鲍曼不动杆菌的耐药监测和抗菌药物临床应用合理性管控,减缓耐药菌株产生。
OBJECTIVE:To probe into the correlation between the application of antibiotics and the drug resistance of acinetobacter baumannii among inpatients in the Central People ' s Hospital of Zhanjiang ( hereinafter referred to as &quot;our hospital&quot;) , so as to provide references for clinical rational medication and infection control . METHODS:The application of antibiotics and drug resistance rates of detected acinetobacter baumannii on frequently -used antibiotics among inpatients in our hospital from 2010 to 2016 were retrospectively collected , SPSS 17.0 was adopted to conduct the correlation analysis .RESULTS:The drug resistance rates of acinetobacter baumannii on piperacillin-tazobactam , cefoperazone-sulbactam , amikacin , meropenem and imipenem presented significantly positive correlation with the daily defined dose ( DDDs ) of imipenem and cefoperazone-sulbactam ( P 〈0.05 ); the drug resistance rate of acinetobacter baumannii on amikacin presented significantly negative correlation with the DDDs of other antibiotics ( P〈0.05 ); the detection rate of acinetobacter baumannii presented significantly positive correlation with the DDDs of piperacillin-tazobactam(r=0.828,P=0.021); the detection rate of multi-drug resistance(MDR) acinetobacter baumannii presented significantly positive correlation with the DDDs of cefoperazone -sulbactam , imipenem and other antibiotics at special use levels ( r=0.850, P=0.015; r=0.868, P=0.011; r=0.895, P=0.006), while presented significantly negative correlation with the DDDs of amikacin (r =-0.845,P =0.017). CONCLUSIONS:The drug resistance rate of acinetobacter baumannii on frequently-used antibiotics in our hospital is at generally high level , the drug resistance rate of acinetobacter baumannii is correlated with the DDDs of antibiotics . Drug resistance monitoring on acinetobacter baumannii and rational application controlling of antibiotics should be reinforced, so as to slow the producing of drug-resistant strain.
