摘要
目的研究大承气汤外敷结合常规治疗重症急性胰腺炎(SAP)的临床效果及对血清C反应蛋白(CRP)、降钙素原(PCT)的影响。方法将2010年6月~2016年2月在汉川市人民医院消化内科就诊的60例SAP患者随机分为对照组和实验组,每组30例。对照组按照重症急性胰腺炎的常规方案治疗,实验组在常规治疗基础上加大承气汤外敷。两组疗程均为14 d。记录患者的腹痛、腹胀缓解时间,肠鸣音恢复时间及排便时间,住院时间、住院费用;检测治疗前后的血淀粉酶(AMY)、CRP、PCT。结果实验组的总有效率为96.67%,高于对照组的83.33%(P<0.05)。实验组的腹痛、腹胀缓解时间,肠鸣音恢复时间及排便时间,血常规,血、尿淀粉酶恢复时间均短于对照组(P<0.01)。实验组的住院时间短于对照组,住院费用低于对照组(P<0.01)。实验组治疗后的CRP、PCT水平,Ranson、APACHEⅡ、CT评分明显低于对照组(P<0.01)。结论大承气汤外敷结合常规方案能明显改善SAP患者的急性期腹痛、腹胀及肠麻痹等临床症状,降低CRP、PCT水平,减轻胰腺炎性的全身损伤,利于疾病的康复。
Objective To study the clinical effect of Dachengqi Decoction as an external application combined with conventional therapy on severe acute pancreatitis （SAP） and its influence on serum C-reactive protein （CRP） and proealeitonin （PCT）.Methods From June 2010 to February 2016,60 SAP patients treated in Department of Gastroenterology in Hanchuan People＇s Hospital were randomly divided into the control group （n=30） and experimental group （n=30）.In the control group,conventional regimen was used,while in the experimental group,besides the conventional regimen, Dachengqi Decoction as an external application was added.The courses of treatment lasted 14 days in the two groups. The remission time of abdominal pain and distension,recovery time of bowel sound,time to defecate,and hospital stay and cost were recorded.The blood amylase （AMY）,CRP,and PCT before and after treatment were tested.Results The overall effective rate was 96.67% in the experimental group,which was higher than that in the control group （83.33%） （P〈0.05）.In the experimental group,the remission time of abdominal pain and distension,recovery time of bowel sound, defecation time,time of blood routine,blood and urine AMY being normal were all shorter compared with those in the control group （P〈0.01）.The hospital stay in the experimental group was shorter than the control group,and the hospitalization cost was lower in the experimental group （P〈0.01）.The level of CRP and PCT and score of Ranson,APACHE Ⅱ ,and CT in the experimental group were significantly lower than those in the control group （P〈0.01）.Conelusion Combination of Dachengqi Decoction as an external application and conventional regimen can remarkably improve clinical symptoms including abdominal pain and distension,and intestinal paralysis,etc.at an acute stage in SAP patients,decrease the level of CPR and PCT,alleviate systemic damage due to inflammation of pancreas,and is beneficial to the re- habilitation of disease.
