摘要
目的:探究中药保留灌肠治疗慢性肾衰的有效性。方法:根据文献的纳入与排除标准制定检索策略,以灌肠、肾衰等为检索词,在CNKI、维普、万方数据库的主题词字段进行检索,筛选出符合要求的随机对照研究,对纳入的文献进行资料提取,方法学质量评价,对所有研究进行合并统计量分析合成,评定中药保留灌肠治疗慢性肾衰的有效性。结果:(1)本研究共纳入研究29篇,其中试验组共1214例患者,对照组共1146例患者,均为中药灌肠联合常规西药治疗与单纯西药治疗比较。对治疗的总有效率进行meta分析,结果示:OR合并=3.60,95%CI=2.95~4.41,P<0.00001,说明中药灌肠联合常规西药治疗治疗慢性肾衰的疗效要优于单纯西药治疗。(2)纳入研究中,23篇以尿素氮为观察指标,对其进行meta分析,结果示:MD=5.00,95%CI(2.17,7.86),P<0.00001,中药灌肠联合常规西药治疗治疗对降低尿素氮水平,疗效要优于单纯西药治疗。(3)纳入研究中,24篇以血肌酐为观察指标,对其进行meta分析,结果示:MD=68.57,95%CI(65.89,71.25),P<0.00001,中药灌肠联合常规西药治疗治疗对降低血肌酐水平,疗效要优于单纯西药治疗。结论:中药保留灌肠联合常规西药治疗慢性肾衰疗效优于单纯西药治疗,对于降低肌酐、尿素氮效果更好。但是纳入文献病例的基本情况、诊断标准、干预措施、结局评价标准均不统一,方法学描述不足以及发表偏倚的存在可能使本研究结果产生偏倚,影响论证强度。
Objective ： To explore the effectiveness of retention enema with traditional Chinese medicine in the treatment of chronic renal failure. Methods ： According to the inclusion and exclusion criteria, the retrieval strategy was established. Trough enema, renal failure as the search term in the search keyword fieldof CNKI, VIP, Wanfang database, randomized controlled trials were screened. The data were extracted, and the methodological quality was evaluated. The statistical analysis was used to evaluate the effectiveness of retention enema with traditional Chinese medicine in the treatment of chronic renal failure. Results ： ①A total of 29 articles were included in this study, including a total of 1214 patients in the experimental group, and a total of 1146 patients in the control group. All the traditional Chinese medicine enema combined with conventional Western medicine treatment compared with simple Western medicine treatment. The total effective rate of treatment was analyzed by recta, the results showed that： OR=3.60,95%CI=2.95-4.41, P〈0.00001, indicating that traditional Chinese medicine enema combined with Western medicine in the treatment of chronic renal failure is better than pure Western medicine treatment. ② The 23 articles included in the study, using urea as observation indexes, the results of meta analysis showed that ： MD=5.00,95%CI （2.17,7.86）, P〈0.00001, traditional Chinese medicine enema combined with conventional Western medicine treatment to reduce the level of urea nitrogen, curative effect was better than pure Western medicine treatment. ③ The 24 articles included in the study, using serum creatinine as observation indexes, the results of meta analysis showed ： MD=68.57,95%CI （ 65.89,71.25 ）, P〈0.00001, traditional Chinese medicine enema combined with conventional Western medicine treatment to reduce the level of serum creatinine, the curative effect was superior to simple Western medicine treatment. Conclusion： Traditional Chinese medicine retention enema combined with conventional Western medicine treatment of chronic renal failure was better than pure Western medicine treatment, thereinto, the effect of downregulation of creatinine and urea nitrogen was best. However, the basic situation, diagnostic criteria, interventions and outcome evaluation criteria were not uniform. The lack of methodological description and publication bias may lead to bias in the results of this study, which may affect the strength of argumentation.
