摘要
目的通过对皮肤科超说明书用药调查分析,促进皮肤科用药的规范管理,同时为进一步开展超说明书用药的相关研究提供参考。方法随机抽取江西省皮肤病专科医院2014年1—12月门诊处方32 684张,参照药品说明书内容判断是否存在超说明书用药情况,对超说明书用药类型进行调查分析。结果 32 684张处方中,存在超说明书用药处方3195张,超说明书用药发生率为9.78%;超说明书用药类型主要包括超适应证(37.33%)、超用法用量(30.89%)、改变给药途径(19.64%)及超年龄(10.95%)。结论对符合循证医学和药学的超说明书情况,医院需进行规范,将临床经典用药但未纳入皮肤病适应证的药物品种进行超说明书用药备案;对超说明书用药行为,应加强处方点评与反馈,进行有效干预,促进皮肤科的合理用药。
Objective To promote the standardized management of medication use in department of dermatology and to provide a reference for further research through investigating and analyzing the off-label medication use.Methods A total of 32 684 outpatient prescriptions written between January and December 2014 in Jiangxi Specialized Dermatology Hospital were randomly selected in this study.The off-label medication use was judged according to the content of medical instructions,and the types of off-label medication use were analyzed.Results Totally 3195 off-label use prescriptions(9.78%)were found among the 32 684 prescriptions.The types of off-label medication use mainly included overindication(37.33%),overdosage(30.89%),administration route change(19.64%)and overage(10.95%).Conclusion The off-label medication use which is confirmed by evidence-based medicine and pharmacy should be standardized,and the classic drugs which are not included in skin disease indications should be registered.In addition,prescription evaluation,feedback and intervention should be strengthened to promote the rational medication use in dermatology.
