摘要
目的探讨体素内不相干运动(IVIM)在小型猪颌下腺放疗损伤中的应用价值。方法将6只雌性广西巴马小型猪随机分成实验组和对照组,将小型猪双侧颌下腺放入照射野内,实验组小型猪照射剂量为20 Gy/每侧颌下腺,对照组小型猪照射剂量为0 Gy/每侧颌下腺。照射前和照射后4周分别对两组小型猪颌下腺行常规MR及双指数模型IVIM扫描,分别观察两组小型猪照射前后以及实验组和对照组之间颌下腺IVIM参数(D值、D~*值和f值)的差异。待MRI检查结束后,处死两组小型猪,取双侧颌下腺标本行HE染色,观察两组小型猪颌下腺组织结构的不同。结果照射前实验组与对照组颌下腺IVIM各参数均无统计学差异,D值分别为(0.855±0.050)×10^(-3)mm^2/s、(0.829±0.041)×10^(-3)mm^2/s;D~*值(18.966±1.323)×10^(-3)mm^2/s、(19.416±1.705)×10^(-3)mm^2/s;f值0.189±0.009、0.194±0.023(P>0.05)。照射后实验组颌下腺IVIM各参数分别为D值(1.007±0.059)×10^(-3)mm^2/s、D~*值(4.788±0.835)×10^(-3)mm^2/s、f值0.394±0.031,与照射前实验组和照射后对照组颌下腺IVIM各参数比较,均存在统计学差异(P<0.05);照射后对照组颌下腺IVIM各参数分别为D值(0.846±0.130)×10^(-3)mm^2/s、D~*值(19.733±3.286)×10^(-3)mm^2/s、f值0.193±0.026,与照射前比较,两者无统计学差异(P>0.05)。实验组与对照组颌下腺病理HE染色结果显示二者明显不同。结论 IVIM可对小型猪颌下腺放疗损伤模型进行无创、量化评估,以期为头颈部放射治疗患者的颌下腺功能损伤评估奠定基础。
Objective To assess the value of intravoxel incoherent motion(IVIM) in radiotherapy injury of submandibular gland in minipigs. Methods 6 female minipigs(Bama County,Guangxi) were randomly divided into control group and experimental group. We Put the bilateral submandibular glands of the minipigs in the radiation field,irradiation doses in experimental groups and control groups were 20 Gy and 0 Gy in each side of the submandibular gland respectively. Routine magnetic resonance(MR) and biexponential model IVIM scan were performed respectively at 4 weeks before radiation and 4 weeks after radiation in submandibular glands in two groups of minipigs. We assessed the difference of the IVIM parameters in submandibular gland of two groups of minipigs in pre-and post-radiation,and to assess difference of the IVIM parameters between control and experimental group. The IVIM parameters include diffusion(D) 、pseudodiffusion(D~*) and perfusionfraction(f). After MR examination,put two groups of minipigs to death. Specimens were taken from bilateral submandibular gland for HE staining. To study the difference of tissue structure in submandibular gland in two groups of minipigs. Results There was no statistically significant of IVIM parameters in submandibular gland between experimental group and control group before radiation. [D values of experimental groups were(0. 855 ± 0. 050) × 10^-3mm^2/s、D values of control groups were(0. 829 ± 0. 041) × 10^-3mm^2/s;D^*values of experimental groups were(18. 966 ± 1. 323) × 10^-3mm^2/s、D~*values of control group were(19. 416 ± 1. 705) × 10^-3mm^2/s;f values of experimental groups were(0. 189 ± 0. 009) 、f values of control groups were(0. 194 ± 0. 023);P〈0. 05]. The IVIM parameters in submandibular gland of experimental group after radiation were the following:D value was(1. 007 ± 0. 059) × 10^-3mm^2/s、D^*value was(4. 788 ± 0. 835) × 10^-3mm^2/s、f value was(0. 394 ± 0. 031). There was statistically significant difference of IVIM parameters in submandibular gland between pre-radiation and postradiation(P〈0. 05),experimental group and control group had statistically significant difference of IVIM parameters after postradiation(P〈0. 05). The IVIM parameters in submandibular gland of control group after radiation were the following:D value was(0. 846 ± 0. 130) × 10^-3mm^2/s、D^*value was(19. 733 ± 3. 286) × 10^-3mm^2/s、f value was(0. 193 ± 0. 026). There was no statistically significant difference between pre-radiation and postradiation in control group(P〈0. 05). we then performed pathology of HE staining. Results were different between experimental group and control group. Conclusion The IVIM can be used in evaluating the radiotherapy damaged model of submandibular gland in minipigs non-invasively and quantitatively. It is promising to lay the foundation for function impairment assessment in submandibular gland in patients with head and neck radiotherapy injury.
