辛伐他汀预处理对脓毒症大鼠急性肾损伤的影响

Effect of simvastatin preconditioning on acute renal injury in septic rats

摘要 目的评价辛伐他汀预处理对脓毒症大鼠急性。肾损伤的影响。方法清洁级健康雌性Wistar大鼠128只,49~63日龄,体重200~250g,采用随机数字表法分为4组:对照组(C组,/2=8)、假手术组(Sham组,n=40)、脓毒症组(Sep组,n=40)和辛伐他汀预处理组(sP组,n=40)。采用盲肠结扎穿孔(CLP)法制备脓毒症模型。sP组CLP前给予辛伐他汀灌胃,1次,d,连续2周;其余各组给予等容量生理盐水灌胃。Sham组、Sep组和sP组随机取8只大鼠记录术后48h内生存情况。Sham组、Sep组和SP组于CLP后3、6、24、48h时随机取8只大鼠、c组于麻醉后取血标本,检测白细胞计数,采用ELISA法检测血清胱抑素-C(Cys—C)浓度;取肾组织,采用免疫组化法确定肾组织C.反应蛋白(CRP)阳性细胞百分比;记录直肠温度。结果c组比较,Sep组和sP组生存率、白细胞计数和直肠温度降低,血清Cys—C浓度和肾组织CRP阳性细胞百分比升高(P〈0.05),Sham组差异无统计学意义(P〉0.05);与Sep组比较,sP组生存率、白细胞计数和直肠温度升高,血清Cys。C浓度和肾组织CRP阳性细胞百分比降低(P〈0.05)。结论辛伐他汀预处理可抑制肾组织炎症反应,减轻脓毒症大鼠急性肾损伤。 Objective To evaluate the effect of simvastatin preconditioning on acute renal injury in septic rats. Methods A total of 128 pathogen-free healthy female Wistar rats, aged 49-63 days, weig- hing 200-250 g, were divided into 4 groups using a random number table: control group (group C, n= 8) , sham operation group (group Sham, n= 40) , sepsis group (group Sep, n= 40) and simvastatin pre- conditioning group (group SP, n = 40). Sepsis was induced by cecal ligation and puncture (CLP). Simv- astatin was injected through a gastric tube into the stomach once a day for 2 consecutive weeks before CLP in group SP, while the equal volume of normal saline was given instead in the other groups. Eight rats were randomly selected from Sham, Sep and SP groups, and the survival rates within 48 h after CLP were recor- ded. Eight rats were randomly selected at 3, 6, 24 and 48 h after CLP for blood sampling in Sham, Sep and SP groups, and blood samples were collected after anesthesia in group C. White blood cells were coun- ted, and the serum cystatin C (Cys-C) concentrations were determined by enzyme-linked immunosorbent assay. Kidney specimens were obtained for determination of the percentage of C-reactive protein (CRP) positive ceils in renal tissues by immunohistochemistry. The rectal temperature was recorded. Results Compared with group C, the survival rates, white blood cell count and rectal temperature were significantly decreased, and the serum Cys-C concentration and percentage of CRP positive cells in renal tissues were in- creased in Sep and SP groups (P〈O. 05), and no significant change was found in the parameters mentionedabove in group Sham (P〉0.05). Compared with group Sep, the survival rates, white blood cell count and rectal temperature were significantly increased, and the serum Cys-C concentration and percentage of CRP positive cells in renal tissues were decreased in group SP (P〈0. 05). Conclusion Simvastatin precondi- tioning can inhibit inflammatory responses of renal tissues and reduce acute renal injury in septic rats.
