一期胃代食管术治疗新生儿长段型食管闭锁的疗效分析 被引量:7

摘要 目的探讨一期胃代食管术治疗新生儿长段型食管闭锁的疗效。方法对2008年3月至2016年6月本院采用胃代食管术治疗的12例新生儿先天性长段型食管闭锁进行回顾性分析,其中男9例,女3例。Ⅰ型2例,Ⅲa型10例。结果所有患儿均顺利完成手术。10例治愈出院,1例死亡,1例家长放弃治疗。术后有6例严重肺炎,2例吻合口瘘。治愈的10例患儿获随访1~6年,其中吻合口狭窄3例,均行食管扩张术治愈;轻度胃食管反流7例,采用少量多餐及体位喂养治疗后症状缓解,未行手术治疗。结论一期胃代食管术在新生儿期治疗长段型食管闭锁临床可行,避免了分期手术,缩短了治疗周期,有助于提高治愈率。 Objective To explore the application of one-stage gastric transposition for long-gap esopha- geal atresia in neonates. Methods A total of 12 neonates of long-gap esophageal atresia undergoing one-stage gastric transposition from March 2008 to lune 2014 were analyzed. There were 9 boys and 3 girls. The diagnosis was confirmed by esophageal radiography. And the clinical types were 1 ( n = 2 ) and m a ( n = 10 ). Results All procedures were completed successfully. Ten cases were cured, I case died and 1 case refused fur- ther interventions after operation. There were postoperative pneumonia ( n = 6 ) and stoma leakage ( n = 2 ). The follow-up period was 1 to 6 years. Three cases of anastomotic stricture were cured by balloon dilation. And 7 cases developed mild gastroesophageal reflux but no anti-reflux measure was needed. And gastroesophageal reflux symptoms were alleviated by smaller meals. Conclusion One-stage gastric transposition is feasible and acceptable for long-gap esophageal atresia in neonates. It may avoid staged procedures, shorten treatment course and improve curative rate.
