摘要
扩张型心肌病(DCM)是一种主要表现为以单侧或双侧心室扩大,同时伴有心肌收缩功能障碍为特征的心肌病,其病因机制尚未完全明确。临床主要以药物治疗为基础,进而改善症状,延缓病情进展。左心室辅助装置、心脏再同步化治疗、植入型心律转复除颤器、心脏移植和基因治疗等方法,因为多种原因无法真正根治DCM。近年来,干细胞移植治疗DCM取得了巨大进展,大量实验研究表明,干细胞在移植后可分化为心肌细胞、血管内皮细胞等,增加心肌收缩力、逆转心室重构、改善心肌供血,进而提高心功能。尽管干细胞的作用机制、转归及移植后远期不良反应尚未完全明确,移植干细胞种类及植入方法选择等问题也存在很多争议,但干细胞移植仍被看作是未来一种非常有前景的DCM治疗手段。
Dilated cardiomyopathy ( DCM) mainly present as unilateral or bilateral ventricular enlargement with myocardial contractile dysfunction at the same time. The etiology and pathogenesis of this disease have not yet been fully understood. Treatment mainly based on pharmacological therapy to improve symptoms and delays its progression. Other treatment include left ventricular assist device, cardiac resynchronization therapy,implantable cardioverter defibrillator,heart transplantation and gene therapy. But for many reasons, DCM cannot be cured. In recent years, great progress has been made in stem cell transplantation for the treatment of DCM. Many experimental studies have shown that stem cells can differentiate into myocardial cells and vascular endothelial cells after transplantation,further lead to increase myocardial contractility,reverse ventricular remodeling,improve myocardial blood supplement,and improve cardiac function. Although the mechanism of stem cell action and long-term adverse effects are not fully understood,and types of stem cells and implantation methods still are controversial,stem cell transplantation is still regarded as a promising DCM therapy in the future.
