摘要
目的:研究膀胱截石位和站立位臀大肌位置的变化及影响因素,探讨膀胱截石位臀大肌肌内注射的精确定位方法。方法:门诊手术患者639例,按10岁年龄段分为20~29岁组、30~39岁组、40~49岁组和50~59岁组,按体质量指数(BMI)≤18.5-、18.5-22.9、23-24.9、25-29.9、≥30分为5组,采用超声技术分别测量患者站立位、膀胱截石位时同侧同部位的臀大肌位置变化,并进行统计学比较分析。结果:在膀胱截石位下同侧同部位的臀大肌位置与站立位相比,改变存在明显差异,统计学计算平均垂直下降3.39cm±1.56cm。并随着患者年龄、BMI的增大,臀大肌下降数值越大,有正相关关系。结论:在膀胱截石位下进行臀大肌注射定位时,可根据患者年龄、BMI等因素,考虑酌情在传统定位区域内,垂直下移1~3.9cm。
Objective: To investigate the variation and factors of and standing position for more precise injection site. Methods: gluteus maximus injection site in the dorsal lithotomy posmon Six hundred and thirty-nine outpatients were enrolled in this study in our hospital. The subjects were stratified according to age and body mass index (BMI). The position of the gluteus maximus was measured in the dorsal lithotomy position and standing position by ultrasonic technique. Then the data were analyzed statistically. Results: Between the dorsal lithotomy position and standing position, there were significant differences (the average vertical decrease was 3. 39-+-1. 56 cm, P%0. 01) in the variation of the gluteus maximus injection site. The variations were correlated with age and BMI. With the increase of age and BMI, the vertical variation of the gluteus maximus was increased with statistically significant difference. Conclusion: To perform gluteus maximus injection in the dorsal lithotomy position, the injection site is lower than the traditional location area (vertical downward 1-3.9 cm) according to the patient's age and BMI.
