摘要
目的:探讨留置肺导管局部治疗联合全身用药治疗脓肿分枝杆菌肺病的临床价值,为以脓肿分枝杆菌为代表的非结核分枝杆菌(NTM)肺病的治疗提供新思路。方法:将深圳市第三人民医院2014年8月至2017年4月收治的25例脓肿分枝杆菌肺病患者分为肺导管组(n=11)和单纯化疗组(n=14)。肺导管组采用留置肺导管局部注药联合全身药物治疗,单纯化疗组单用全身药物治疗,观察强化期结束后的近期疗效。结果:强化期结束后,肺导管组痰菌转阴率72.73%、病灶吸收率81.81%明显高于单纯化疗组的28.57%、21.43%,组间比较,差异具有统计学意义(P<0.05)。肺导管组无咯血、胸痛等严重不良反应。结论:留置肺导管局部注药联合全身用药治疗脓肿分枝杆菌的近期疗效有提高,操作安全,不良反应少。
Objective To evaluate the clinical treatment value of local drug administration by catheter in treatment of mycobacterium abscessus pulmonary disease. Methods From Oct. 2014 to Apr. 2017, 25 patients with mycobacterium abscessus pulmonary disease were divided into experimental and control groups. 11 patients in experimental group received local drug administration by catheter combined with general anti-nontuberculosis treatment. And 14 patients in control group received general anti-nontuberculosis treatment alone. The short-term clinical treatment outcomes were observed. Results At the end of intensifying treatment, the sputum negative conversion rates were 72.73 % and 28.57 % in the experimental and control groups respectively; and the foci absorption rates were 81.81 % and 21.43 % in the experimental and control groups respectively. There were statistically significant differences on the sputum negative conversion rates and foci absorption rates in two groups（P〈0.05）. There were no severe adverse reactions observed in the experimental group. Conclusion The short-term clinical treatment outcome of local drug administration by catheter in treatment of mycobacterium abscessus pulmonary disease was better than that of the general anti-nontuberculosis treatment alone. The combined treatment approach is safe and has few side effects.
