摘要
目的探讨腹腔镜下子宫肌瘤剔除术和传统开腹子宫肌瘤剔除术治疗子宫肌瘤患者时局部微循环和炎症情况的差异。方法选取2013年5月至2015年6月就诊的子宫肌瘤患者90例,随机分为观察组和对照组各45例。观察组采取腹腔镜下子宫肌瘤剔除术,对照组采取开腹子宫肌瘤剔除术。观察并记录术前和术后1 d、3 d、7 d微循环指标和炎性细胞因子的变化。结果术前两组患者血清的C-反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子α(TNF-α)含量无显著差异(P>0.05);而术后1 d、3 d、7 d观察组的CRP、IL-6、TNF-α含量显著少于对照组(P<0.05);术前两组患者的微循环指标无显著差异(P>0.05),术后1 d、3 d、7 d观察组血流灌注、红细胞聚集指数异常率、甲襞微循环中的管襻形态积分和血液流态积分值显著低于对照组(P<0.05);且术后1 d、3 d、7 d观察组毛细血管管径、宫颈血管清晰率显著高于对照组(P<0.05)。结论腹腔镜下子宫肌瘤剔除术对机体的应激、创伤更小,微循环功能更好,有利于患者术后的恢复,值得在临床上推广应用。
Objective To explore the difference of local microcirculation and inflammatory condition in patients with uterine fibroids with laparoscopic hysteromyoma removal and traditional open abdominal hysteromyoma.Methods 90 cases of uterine fibroids were selected from May2013 to June 2015,and were randomly divided into observation group and control group,45 cases in each group.Laparoscopic hysteromyoma removal was performed in the observation group,and the control group was treated with an open abdominal myoma.To observe and record the changes of 1 d,3 d,7 d microcirculation index and inflammatory cytokines before and after operation.Results The CRP,IL-6 and tnf-alpha in the serum of the two groups preoperative had no significant difference(P > 0.05).The contents of CRP,IL-6 and tnf-alpha in the 1 d,3 d and 7 d observation groups were significantly lower than those in the control group(P < 0.05).Preoperative microcirculation indexes of two groups of patients had no significant difference,postoperative observation group 1 d,3 d,7 d and nailfold microcirculation perfusion,erythrocyte aggregation rate in the tube loop form and the blood flow integral value were significantly lower than those of the control group(P < 0.05).After surgery,the capillary tube and cervical vascular articulation of the capillary tube and cervix were significantly higher than that in the control group(P < 0.05).Conclusion Laparoscopic uterine fibroids eliminate stress on the body,less trauma,microcirculation function better,is beneficial to the recovery of patients with postoperative,its clinical application value is high,and worth popularizing in clinic.
