奥美沙坦对老年高血压患者血清肌酸激酶同工酶、心肌肌钙蛋白Ⅰ及颈动脉粥样硬化的影响 被引量:2

摘要 目的:研究奥美沙坦对老年高血压患者心肌缺血灌注及颈动脉粥样硬化进展的影响。方法:将124例老年高血压患者随机分为观察组和对照组,各62例。观察组患者采用奥美沙坦进行治疗,对照组患者采用钙通道阻滞剂进行治疗,比较两组患者的血清肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)及颈动脉粥样硬化程度。结果:与治疗前比较,两组患者治疗后血压均降低(P<0.05);治疗后,两组患者血压值比较,差异无统计学意义(P>0.05);观察组患者治疗后血清CK-MB及cTnI浓度均低于同组治疗前(P<0.05),并且低于对照组治疗后(P<0.05);与治疗前相比,两组患者治疗后颈总动脉后壁内-中膜厚度(IMT)、颈总动脉血流阻力指数(RI)均降低(P<0.05),斑块稳定性均升高(P<0.05);观察组治疗后IMT、RI均低于对照组治疗后(P<0.05),斑块稳定性高于对照组治疗后(P<0.05)。结论:奥美沙坦治疗老年高血压,在有效控制血压的基础上,能够提高颈动脉粥样斑块稳定性,有一定临床应用价值。 Objective:To investigate the effects of olmesartan on creatine kinase - MB ( CK - MB), cardiac troponin I (cTnI) and carotid atheroselerosis in elderly hypertensive patients. Methods :124 elderly hypertensive patients were randomly divided into the observation group and the control group, 62 cases in each group. The observation group was treated by olmesartan while the control group was treated by calcium antagonists, with the CK -MB, cTnI and the degrees of carotid atherosclerosis compared. Results:Compared with that before treatment, the blood pressure in both groups after treatment was decreased ( P 〈 0.05 ) ; after treatment , there was no difference in blood pressure between the two groups ( P 〉 0.05 ) ; the levels of CK - MB and nTnl in the observation after treatment were lower than those before treatment ( P 〈 0.05 ) and those in the con- trol group after treatment ( P 〈0.05) ; IMT and RI were decreased in both groups after treatment ( P 〈0.05) and plaque stability was better in both groups after treatment ( P 〈 0.05 ) ; IMT and RI in the observation group after treatment were more significantly decreased than those in the control group ( P 〈 0.05 ) and plaque stability of the observation group was significantly better than that before treatment and the control group ( P 〈 0.05 ). Conclusion : The use of olmesartan in the treatment of elderly hypertensive patients can effectively control blood pressure, repair myocardial function and carotid atherosclerosis, worthy of clinical application.
