摘要
目的观察经尿道前列腺等离子双极电切术(plasmakinetic resection of prostate,PKRP)治疗老年良性前列腺增生症的近期疗效,并探讨前列腺组织的切除比率对术后近期疗效的影响。方法回顾性分析2014年1月至2016年12月福建医科大学附属泉州第一医院收治的93例符合入组标准的老年良性前列腺增生(benign prostatic hyperplasia,BPH)的临床资料。入组患者均接受PKRP治疗,计算切除的前列腺体积(resection of Prostate volume,RPV)。治疗前、后3个月进行国际前列腺症状评分(international prostate symptom score,IPSS)、生活质量评分(quality of lifescore,QOL)及尿动力学指标比较。根据上述测定结果将临床疗效分为效果满意组与效果不满意组,分析TPV、TZV、RPV、RPV/TPV、RPV/TZV对手术效果的影响。结果术后3个月IPSS评分、QOL评分、PVR较术前降低(P<0.01),Qmax、MCC、BC较术前升高(P<0.01)。术后效果满意组治疗后3个月IPSS评分、QOL评分低于效果不满意组(P<0.05),Qmax、MCC、BC高于效果不满意组(P<0.01)。术后效果满意组RPV/TPV、RPV/TZV均高于效果不满意组(P<0.01)。RPV/TPV、RPV/TZV对PKRP手术效果预测价值的应用受试者工作特征曲线(ROC)结果显示,RPV/TPV的曲线下面积为0.878(P<0.01),RPV/TZV的曲线下面积为0.811(P<0.01)。结论 PKRP是治疗老年良性前列腺增生症的安全、有效的手术方式,可改善患者临床症状及尿动力学参数,提高患者生活质量,术后近期疗效与术中前列腺组织的切除比率存在相关性。
Objective To observe the short-term efficacy of plasmakinetic resection of prostate in the treatment of elder patients with benign prostatic hyperplasia（BPH）,and to explore the impact of prostate resection volume on the postoperative short-term efficacy. Methods A retrospective analysis was conducted on the clinical data of 93 elderly patients with BPH who met the inclusion criteria and received PKRP treatment.The resection of prostate volume（RPV）was calculated.The international prostate symptom score（IPSS）,quality of life score（QOL）and urodynamic index were compared before and after 3 months of treatment. Results The IPSS score,QOL score and PVR were significantly lower 3 months post-surgery as well as among patients with satisfactory clinical outcome（P〈0.01）,while the Qmax,MCC and BC were significantly higher（P〈0.01）.The RPV/TPV and RPV/TZV ratios were also significantly higher among patients with satisfactory outcome compared with those who were not（P〈0.05）.The predictive values of RPV/TPV and RPV/TZV on PKRP by the receiver operating characteristic curve（ROC）showed that the area under the curve of RPV/TPV was 0.878（P〈0.01）,and the area under the curve of RPV/TZV was 0.811（P〈0.01）. Conclusions PKRP is a safe and effective method in the treatment of elderly patients with BPH,which can significantly improve the clinical symptoms and urodynamic parameters and improve the quality of life of patients,its short-term curative effect is related to the resection volume of intraoperative prostate tissue.
