摘要
目的:探讨磁共振扩散张量成像(Diffusion tensor imaging,DTI)在前列腺癌(Prostate cancer,PCa)病理分级中的应用价值。方法:回顾性分析40例PCa患者的临床资料,应用Gleason评分进行分级。患者均行DTI检查,测定各向异性分数(Fractional anisotropy,FA)和表观弥散系数(Apparent diffusion coefficient,ADC),分析FA值、ADC值与患者Gleason评分的关系。结果:低级别组DTI图表现为纤维束排列稠密、未见中断;中级别组DTI图显示纤维束排列紊乱、稀疏;高级别组DTI图显示纤维束中断;低级别组、中级别组和高级别组FA值逐渐升高,ADC值逐渐减小(P<0.05);ADC值和Gleason评分呈负相关,FA值和Gleason评分呈正相关(P<0.05)。结论:磁共振扩散张量成像可作为无创评估PCa患者病理分级的重要指标。
Objective: To investigate application values of magnetic resonance diffusion tensor imaging( DTI) in pathological grading of prostate cancer( PCa). Methods: The clinical data of 40 patients with PCa were retrospectively analyzed. The Gleason score was used for grading. All the patients underwent DTI examination. The fractional anisotropy( FA) and apparent diffusion coefficient( ADC) were measured. The relationships among the FA value,ADC value and patient Gleason score were analyzed. Results: The DTI patterns in the low-grade group showed dense fiber bundles with no disruption; the middle-grade DTI patterns showed disarranged and sparse fiber bundles; and the high-grade group DTI showed interrupted fiber bundles. The FA values of the low-grade,middle-grade and high-grade groups gradually increased,while the ADC values gradually decreased( P<0.05). Further,the FA value was positively correlated with Gleason score,while the ADC value was negatively correlated with Gleason score( P<0.05). Conclusions: DTI can be used as a noninvasive and an important assessment tool for PCa pathological grading.
