血清骨碱性磷酸酶和25羟维生素D水平与婴幼儿维生素D营养状态的相关性 被引量:13

Correlation analysis of serum bone alkaline phosphatase and 25-hydroxyvitamin D levels with vitamin D nutritional status in infants

摘要 目的探讨血清骨碱性磷酸酶(BALP)和25羟维生素D[25-(OH)D]水平与婴幼儿维生素D营养状态的关系。方法选取2013年4月至2016年6月苏州市立医院东区儿科收治的186例确诊为佝偻病的婴幼儿作为病例组,同期300例体检正常的健康婴幼儿作为健康对照组。比较两组受检者血清25-(OH)D和BALP水平,并用Pearson线性相关分析血清BALP与25-(OH)D的相关性。结果病例组患儿血清25-(OH)D不足或缺乏的比例、BALP>250 U/L的比例与健康对照组比较差异均有统计学意义(P<0.05);病例组患儿中不同年龄的患儿血清25-(OH)D不足或缺乏的比例比较差异均有统计学意义(P<0.05),而BALP>250 U/L的比例比较差异无统计学意义(P>0.05);病例组与健康对照组的25-(OH)D水平与BALP水平均无相关性(P>0.05)。结论 25-(OH)D是反映婴幼儿维生素D营养状态的灵敏指标,但血清BALP尚不能作为评价婴幼儿维生素D营养状态的可靠指标。 Objective To explore the correlation of serum bone alkaline phosphatase(BALP) and 25-hydroxyvitamin D 25-(OH)D levels with vitamin D nutritional status in infants.Methods A total of 186 cases of 0-3-year-old children with rickets,who admitted to Department of Pediatrics of our hospital from Apr.2013 to Jun.2016,were selected the case group,and 300 healthy children with normal physical examination were selected as the healthy control group during the corresponding period.The levels of serum 25-(OH)D and BALP were compared between the two groups,and the correlation between serum BALP and 25-(OH)D was analyzed by Pearson linear correlation analysis.Results There were significant differences between the case group and the healthy control group in the proportion of insufficiency or absence of serum 25-(OH)D and the ratio of BALP250 U/L(P0.05).There were significant differences in the proportion of serum deficiency or absence among the children of different ages in the case group(P0.05),while the ratio of BALP250 U/L were not statistically significant(P0.05).There was no correlation between 25-(OH)D levels and BALP levels in the case group and the healthy control group(P0.05).Conclusion The serum 25-(OH)D level is a sensitive index reflecting the nutritional status of vitamin D in children.However,BALP could not be used as a reliable index to assess the nutritional status of vitamin D in infants.
