术前标准b值DWI信号及ADC值与脑膜瘤病理分级分型的关系 被引量:7

摘要 目的探讨术前标准b值DWI信号及ADC值与脑膜瘤病理分级分型的关系。方法对湛江市第二中医医院2013年1月至2017年3月107例接受手术治疗的脑膜瘤患者的术前MRI图像进行回顾性分析,将DWI信号与测量所得ADC值与术后病理结果进行对比分析。结果 107例脑膜瘤患者最终确诊良性98例(91.59%),恶性9例(8.41%);主要亚型包括纤维型40例(37.38%),上皮型33例(30.84%),过渡型14例(13.08%),血管瘤型8例(7.48%),微囊型3例(2.80%),非典型9例(8.41%);良恶性患者的DWI信号强度比较差异无统计学意义(P>0.05);主要亚型DWI信号强度平均秩次为微囊型>纤维型>上皮型>过度型=血管瘤型>非典型,但差异无统计学意义(P>0.05);良性脑膜瘤的ADC_(min)、ADC_(max)、ADC_(avg)值均高于恶性脑膜瘤,但差异无统计学意义(P>0.05);主要亚型中,血管瘤型的ADCmin、ADCmax、ADCavg值均显著大于其他亚型,差异均有显著统计学意义(P<0.01)。结论术前标准b值DWI信号与ADC值无法对脑膜瘤病理分级做出有效区分,但DWI信号联合ADC值仍可对部分特殊脑膜瘤亚型做出区分。 Objective To study the relationship between diffusion weighted imaging (DWI) signal and appar-ent diffusion coefficient (ADC) value under standard b value before operation and pathological classification of meningi-oma. Methods Preoperative MRI images of 107 patients with meningiomas who underwent surgery in the SecondTCM Hospital of Zhanjiang City from January 2013 to March 2017 were retrospectively analyzed. The results of DWIsignal and ADC value were compared with the pathological results after operation. Results Among the 107 patients, 98were finally diagnosed as benign (91.59%), and 9 were confirmed malignant (8.41%). The main subtypes included fibertype (40 cases, 37.38%), epithelial type (33 cases, 30.84%), transitional type (14 cases, 13.08%), hemangioma type (8cases, 7.48%), microcystic type (3 cases, 2.80%), and atypical type (9 cases, 8.41%). DWI signal intensity showed no sta-tistically significant difference between patients with benign and malignant meningioma (P〉0.05). The average rank ofDWI signal intensity of the main subtypes was microcystic type〉fiber type〉epithelial type〉transitional type=hemangio-ma type〉atypical type, but the difference was not statistically significant (P〉0.05); ADCmin, ADCmax, ADCavg of benign me-ningiomas were higher than those of malignant meningioma, but the difference was not significant (P〉0.05). ADCmin, AD-Cmax, ADCavg of hemangioma type were significantly higher than those of other subtypes (P〈0.01). Conclusion TheDWI signal and ADC value under standard b value before operation can not distinguish the pathological grading of me-ningioma, but DWI signal combined with ADC value can still distinguish some special meningioma subtypes.
