摘要
目的分析甲状腺冰冻切片与石蜡切片病理检查对甲状腺疾病的诊断价值。方法选取凉山州会理县人民医院2016年3月—2017年5月收治的甲状腺疾病手术患者88例,均进行甲状腺冰冻切片(观察组)与石蜡切片(对照组),比较两种诊断所需时间及病理诊断结果。结果观察组诊断所需时间短于对照组(P<0.05)。88例患者中,对照组病理诊断结果:恶性甲状腺肿瘤36例(甲状腺髓样癌3例、甲状腺滤泡状癌10例、甲状腺乳头状癌23例),良性甲状腺肿瘤15例,非甲状腺肿瘤37例。观察组病理诊断结果:恶性甲状腺肿瘤33例(甲状腺髓样癌2例、甲状腺滤泡状癌9例、甲状腺乳头状癌22例),良性甲状腺肿瘤14例,非甲状腺肿瘤37例。两组误诊率、延迟诊断率、检出率比较,差异无统计学意义(P>0.05)。结论对于甲状腺疾病患者,术中冰冻切片进行病理诊断具有一定的临床价值,可提高甲状腺肿瘤检出率,缩短病理诊断所需时间,有利于术中及时进行治疗,改善患者预后,但若诊断过程中存在疑问,应待石蜡切片检查结果再确定下一步的治疗方案。
Objective To analyze the diagnostic value of frozen section and paraffin section of thyroid in the thyroid diseases.Methods A total of 88 cases of patients with thyroid diseases were selected from March 2016 to May 2017 in Huili People＇s Hospital of Liangshan Prefecture,all patients were given frozen section（observation group） and paraffin section（control group）.The diagnostic required time and the results of pathological diagnosis were compared between the two groups.Results The diagnostic required time of observation group was shorter than control group（P〈0.05）.In 88 cases of patients,the control group of pathological diagnosis results were 36 cases of malignant thyroid tumor（36 cases of medullary thyroid carcinoma,10 cases of follicular thyroid carcinoma,23 cases of papillary thyroid carcinoma）,15 cases of benign thyroid tumor,37 cases of non-thyroid tumors.The observation group of pathological diagnosis results were 33 cases of malignant thyroid tumor（2 cases of medullary thyroid carcinoma,9 cases of follicular thyroid carcinoma,22 cases of papillary thyroid carcinoma）,14 cases of benign thyroid tumor,37 cases of non-thyroid tumors.No significant differences of misdiagnosis rate,delayed diagnosis rate or detection rate was found between the two groups（P〈0.05）.Conclusion As for thyroid diseases patients,pathological diagnosis of frozen section has a certain clinical value,it can effectively promote the detection rate of diseases,shorten diagnostic required time,which is benefit for treatment,and improve the prognosis of patients.But if the diagnostic process in doubt,we should stay the paraffin section of the results to determine the next step of treatment plan.
