摘要
目的探讨复方荆芥熏洗剂联合肛泰软膏治疗痔疮的临床疗效。方法选取2015年9月—2016年12月上海市第一人民医院宝山分院肛肠科收治并接受混合痔外剥内扎术的患者84例作为研究对象,将所有患者随机分为对照组和治疗组,每组各42例。对照组使用0.02%高锰酸钾溶液坐浴15 min,然后外涂肛泰软膏,2次/d。治疗组给予复方荆芥熏洗剂,10 g加入到沸水1 500 m L中,趁热熏蒸肛门5 min,待温度降至37~40℃时坐浴10 min,然后外涂肛泰软膏,2次/d。两组患者均连续治疗2周。观察两组的临床疗效,比较两组的肛周水肿评分、疼痛指数、创面渗液评分以及疼痛消退时间和创面愈合平均时间。结果治疗后,对照组和治疗组的总有效率分别为97.62%、100.00%,两组比较差异无统计学意义。治疗7、14 d后,两组肛周水肿评分均显著下降,同组与治疗3 d后比较差异有统计学意义(P<0.05);且治疗组肛周水肿评分明显低于同期对照组,两组比较差异具有统计学意义(P<0.05)。治疗3、7 d后,两组疼痛指数评分、创面渗液评分均显著下降,同组治疗前后比较差异有统计学意义(P<0.05);且治疗组这些观察指标明显低于同期对照组,两组比较差异具有统计学意义(P<0.05)。治疗后,治疗组疼痛消退、创面愈合平均时间均显著短于对照组,两组比较差异具有统计学意义(P<0.05)。结论复方荆芥熏洗剂联合肛泰软膏治疗痔疮具有较好的疗效,可明显减轻术后肛门疼痛、水肿的程度,减少创面分泌物,缩短术后疼痛时间,加速伤口愈合,具有一定的临床推广应用价值。
Objective To investigate the clinical efficacy of Compound Jingjie Smoked Lotion combined with Gangtai Ointment in treatment of postoperative mixed hemorrhoids. Methods Patients（84 cases） with mixed hemorrhoids milligan in Department of Proctology of Baoshan Branch of Shanghai First People＇s Hospital from September 2015 to December 2016 were randomly divided into control and treatment groups, and each group had 42 cases. Patients in the control group were given hip bath for 15 min by 0.02% potassium permanganate solution, and then coated with Gangtai Ointment, twice daily. Patients in the treatment group were given Compound Jingjie Smoked Lotion, 10 g added into boiling water 1 500 m L, steaming anus for 5 min, hip bath for 10 min when the temperature dropped to 37 to 40 ℃, and then coated with Gangtai Ointment, twice daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacies were evaluated, and perianal edema score, pain index, wound exudate score, the average times of pain subsides and wound healing in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 97.62% and 100.00%, respectively, and there was no difference between two groups. After treatment for 7 and 14 d, the perianal edema scores in two groups were significantly decreased, and the difference was statistically significant in the same group（P〈0.05）. And the perianal edema score in the treatment group was significantly lower than that in the control group, with significant difference between two groups（P〈0.05）. After treatment for 3 and 7 d, pain index scores and wound exudate scores in two groups were significantly decreased, and the difference was statistically significant in the same group（P〈0.05）. And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups（P〈0.05）. After treatment, the average times of pain subsides and wound healing in the treatment group were significantly shorter than those in the control group, and there was difference between two groups（P〈0.05）. Conclusion Compound Jingjie Smoked Lotion combined with Gangtai Ointment has clinical curative effect in treatment of hemorrhoids, can obviously reduce the degree of postoperative anal pain and edema, reduce wound secretion, shorten the postoperative pain time, accelerate wound healing, which has a certain clinical application value.
