摘要
目的探讨血清生化指标同型半胱氨酸(Hcy)、高敏C反应蛋白(hs-CRP)、缺血修饰白蛋白(IMA)与短暂性脑缺血发作(TIA)的相关性。方法选取2015年10月~2017年3月本院收治的TIA患者126例,按照ABCD^2评分分为3组,低危组42例,中危组43例,高危组41例,另外选取同期健康体检者20例为对照组。收集各组一般临床资料,比较各组血清Hcy、hs-CRP、IMA水平。结果与对照组比较,低危组、中危组和高危组TC、TG、LDL、Hcy、hs-CRP及IMA水平明显升高,HDL水平明显降低(P<0.05)。与低危组比较,中危组和高危组TC、TG、LDL、Hcy、hs-CRP及IMA水平明显升高,HDL水平明显降低(P<0.05)。高危组TC、TG、LDL、Hcy、hs-CRP及IMA水平明显高于中危组,且HDL水平明显低于中危组,差异有统计学意义(P<0.05)。TIA患者血清Hcy、hs-CRP及IMA水平与ABCD^2评分呈正相关(r=0.36,r=0.31,r=0.24,P<0.05),且hs-CRP、Hcy、IMA之间无相关性(P>0.05)。多因素logistic回归分析显示,高脂血症(95%CI:1.371~10.556,P=0.00)、Hcy水平(95%CI:1.064~1.915,P=0.02)是TIA发病的危险因素。结论血清Hcy、hs-CRP、IMA水平与ABCD^2评分呈正相关,高脂血症、Hcy水平是TIA发病的危险因素,测定血清Hcy、hs-CRP、IMA水平有利于病情评估。
Objective To study the association between Hcy,hs-CRP,IMA and transient ischemic attack(TIA).Methods One hundred and twenty-six TIA patients were divided into low risk group(n=42),moderate risk group(n=43)and high risk group(n=43)according to their ABCD^2 score with 20 healthy subjects undergoing physical examinarion served as control group.Their clinical data were recorded and their serum Hcy,IMA and hs-CRP levels were compared.Results The serum levels of TC,TG,LDL,Hcy,IMA and hs-CRP were significantly higher while those of HDL were significantly lower in low risk group,moderate risk group and high risk group than in control group(P0.05),in moderate risk group and high risk group than in low risk group(P0.05),and in high risk group than in moderate risk group(P0.05).The serumlevels of Hcy,hs-CRP and IMA were positively associated with ABCD^2 score in TIA patients(r=0.36,r=0.31,r=0.24,P0.05)but not associated with each other(P0.05).Multivariate logistic regression analysis showed that hyperlidemia and Hcy were the risk factors for TIA(P0.05,P0.01).Conclusion Serum Hcy,hs-CRP,IMA levels are positively associated with ABCD^2 score.Hyperlipidemia and Hcy are the risk factor for TIA.Measurement of serum Hcy,hsCRP,IMA levels is beneficial to the assessment of TIA.
