摘要
目的研制一种新型血浆灌流树脂,探讨同时清除内毒素和炎性细胞因子有效性。方法自制HB-H-7血浆灌流树脂(苯乙烯-二乙烯基苯大孔树脂)。对内毒素水溶液、炎性细胞因子[肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-1、IL-6、IL-8]水溶液进行吸附研究。在此基础上,采用大肠杆菌和内毒素标准品两种物质刺激新鲜血液,模拟内毒素血症产生过程,采用最佳刺激方式处理样本后进行血浆吸附,研究该树脂对内毒素和炎性细胞因子吸附的有效性和安全性。结果 HBH-7树脂对血浆内内毒素和炎性细胞因子(TNF-α、IL-1、IL-6、IL-8)比水溶液内的吸附率低,分别为(52.8±2.3)%、(45.1±3.4)%、(8.2±2.6)%、(36.5±3.7)%和(16.1±2.9)%;对红细胞、血小板和白细胞的吸附率分别为(5.3±3.2)%、(1.9±1.7)%和(38.1±4.3)%;对血浆总蛋白、白蛋白和球蛋白吸附率分别为(13.6±2.9)%、(11.2±2.3)%和(17.5±3.1)%,对血浆电解质吸附率较小。结论 HB-H-7血浆灌流树脂能有效吸附内毒素和炎性细胞因子,有望用于治疗内毒素血症。
Objective To develop a new kind of plasma perfusion resins, and explore the effectiveness of simultaneous re-moval of endotoxin and inflammatory cytokines. Methods The self-made HB-H-7 plasma perfusion resin(styrene-divinylbenzene macroporous resin) was used to study the adsorption property of endotoxin, inflammatory cytokine tumor necrosis fac-tor-α(TNF-α), interleukin(IL)-1, IL-6 and IL-8 in pure water. The E. coli and endotoxin standard product were adopted to stimulate fresh blood, and simulated endotoxemia production process. The plasma adsorption was performed by the best stimu-lated sample method, and then the effectiveness and safety in the adsorption of endotoxins and inflammatory cytokines were tested. Results The adsorption rate of HB-H-7 resin on endotoxin and inflammatory cytokine(TNF-α, IL-1, IL-6, IL-8) in plasma was lower than that in pure water, and the adsorption rate was(52.8 ± 2.3) %,(45.1 ± 3.4) %,(8.2 ± 2.6) %,(36.5 ± 3.7) %and(16.1 ± 2.9) %, respectively. The adsorption rate of HB-H-7 resin on red blood cells, platelets and white blood cells was(5.3 ± 3.2) %,(1.9 ± 1.7) % and(38.1 ± 4.3) %, respectively. The adsorption rate of HB-H-7 resin on plasma total protein, albumin and globin was(13.6 ± 2.9) %,(11.2 ± 2.3) % and(17.5 ± 3.1) %, respectively, and smaller plasma electrolyte adsorption rate was observed. Conclusion It is demonstrated that the HB-H-7 plasma perfusion resin can effectively adsorb endotoxin and inflammatory cytokines, and it is expected to be used for the treatment of endotoxemia.
