限制性液体复苏对创伤休克患者外周血单个核细胞TLRs/NF_(-κ)b信号通路及血清SOD、AOPP水平的影响 被引量:7

Effects of Restricted Liquid Resuscitation on the TLRs/NF-κb Signaling Pathway and Serum SOD and AOPP Levels in Peripheral Blood Mononuclear Cells of Patients with Traumatic Shock

摘要 目的:探讨限制性液体复苏(LFR)对创伤休克患者外周血单个核细胞(PBMC)中TLRs/NF_(-κ)b信号通路表达及血清中SOD、AOPP水平的影响。方法:将我院收治的112例创伤休克患者随机分为对照组和观察组,每组56例。对照组给予积极液体复苏,观察组采用LFR。观察并比较两组患者的临床疗效、液体复苏相关指标、TLRs/NF_(-κ)b信号通路表达及血清SOD、AOPP水平。结果:治疗后,观察组治愈率(85.7%)显著高于对照组(69.6%),死亡率(14.3%)明显低于对照组(30.4%)(P<0.05);复苏30 min、复苏60min,观察组Pa O2、血氧饱和度、血清SOD水平均明显高于对照组同期,血乳酸、AOPP水平、外周血单个核细胞TLR2、TLR9、NF_(-κ)b m NRA表达均明显低于对照组同期(P<0.05)。结论:LFR对创伤休克患者的临床效果优于积极液体复苏,可能与其降低外周血单个核细胞TLRS/NF_(-κ)b信号表达,提高血清SOD水平,降低血清AOPP水平有关。 Objective: To investigate the effect of restrictive fluid resuscitation(LFR) on the TLRs/NF_(-κ)b signaling pathway in peripheral blood mononuclear cells(PBMC) and the serum SOD and AOPP levels of patients with traumatic shock. Methods: 112 patients with traumatic shock were randomly divided into the control group and the observation group, 56 cases in each group. The control group received positive fluid resuscitation, the observation group was given LFR. The clinical efficacy, fluid recovery index, TLRs/NF_(-κ)b signaling pathway and serum SOD and AOPP levels were observed and compared between the two groups. Results: The cure rate of observation group(85.7%) was higher than that of the control group(69.6%), and the mortality rate(14.3%) was lower than that of the control group(30.4%)(P0.05). At 30 min and 60 min after recovery, the level of Pa O2, oxygen saturation of observation group were higher than those of the control group, the blood lactate level was lower than that of that of the control group(P0.05). At 30 min and60 min after recovery, the expression of TLR2, TLR9 and NF_(-κ)b m NRA of observation group were significantly lower than those of the control group(P 0.05). After 30 min and 60 min of recovery, the SOD level of observation group was higher than that of the control group, the level of AOPP was lower than that of the control group(P 0.05). Conclusion: LFR is effective on the patients with traumatic shock, which can improve the expression of TLRS/NF_(-κ)b and inhibit the level of oxygen free radicals.
