摘要
目的:探讨经阴道彩色多普勒超声在子宫切口妊娠中的诊断及治疗价值。方法:选取我院2017-01~10收治的剖宫产术后子宫切口妊娠患者96例,随机分为观察组和对照组,每组48例。对照组给予经腹超声检查,观察组给予经阴道超声检查,比较两种检查方式对子宫切口妊娠的诊断符合率、声像图特征、治疗前后切口肌层厚度及周边血流阻力指数。结果:观察组检出单纯孕囊型24例,不均质团块型22例,超声诊断符合率为95.83%;对照组检出单纯孕囊型21例,不均质团块型19例,超声诊断符合率为83.33%,差异有统计学意义(P<0.05)。两组患者超声声像图分型无明显差异,治疗前后切口肌层厚度及周边血流阻力指数(RI)比较,差异有统计学意义。两组患者均得到有效治疗,未出现子宫切除病例。结论:经阴道彩色多普勒超声在诊断子宫切口妊娠和评估治疗效果中具有重要作用。
Objective：To explore the value of transvaginal color Doppler ultrasonography in the diagnosis and treatment of uterine incision pregnancy.Methods：Ninety-six patients with uterine incision scar pregnancy admitted from January 2017 to October 2017 were divided into observation group and control group,48 cases in each group.Observation group were given transvaginal ultrasound examination,control group were given transabdominal ultrasound examination.Two inspection ways of uterine incision pregnancy diagnosis coincidence rate,ultrasonographic features and the thickness of the anterior incision and the index of peripheral blood resistance were compared.Results：There were 24 cases of pure pregnancy cystic type,22 cases of heterogeneous mass type in observation group,ultrasonic diagnosis rate was 95.83%.There were 21 cases of pure pregnancy cystic type,19 cases of heterogeneous mass type in control group,and ultrasonic diagnosis rate was 83.33%.There were no significant difference in ultrasonography between the two groups of patients,and the difference between the thickness of the anterior incision and the index of peripheral blood resistance was statistically significant.All patients received effective treatment and no hysterectomy was reported.Conclusion：The vaginal color Doppler ultrasound in the diagnosis of uterine incision pregnancy and judgment plays an important role in the treatment effect.
