摘要
目的探讨氟康唑联合硝酸咪康唑治疗真菌性外耳道炎的临床疗效及安全性。方法选取我院耳鼻喉科收治的120例真菌性外耳道炎患者,收集时间2014年1月至2016年12月,其中60例患者采用氟康唑联合硝酸咪康唑治疗(联合组)、60例采用硝酸咪康唑治疗(对照组),疗程2周,对比两组的临床疗效。结果治疗前,联合组和对照组的血清TNF-ɑ、IL-2、IL-4、IL-6水平差异无统计学意义(P>0.05);治疗后,研究组的血清TNF-ɑ、IL-2、IL-4、IL-6水平显著的低于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者的血清TNF-ɑ、IL-2、IL-4、IL-6水平较本组治疗前均显著的降低,差异有统计学意义(P<0.05)。研究组显效率80.00%、有效率16.67%、无效率3.33%,对照组显效率61.67%、有效率30.00%、无效率8.33%,两组比较差异有统计学意义(P<0.05)。研究组有4例(6.67%)患者发生不良发应,对照组仅有1例出现轻度恶心(1.67%),但是两组不良反应发生率差异无统计学意义(P>0.05)。结论氟康唑联合硝酸咪康唑治疗真菌性外耳道炎效果较好,具有更好的抗炎效果。
Objective To discuss the clinical efficacy and safety of fluconazole combined with miconazole nitrate in the treatment of fungal external otitis.Methods Select 120 patients with fungal otitis externa in the otolaryngology from January 2014 to December 2014,among whom there were 60 cases treated by fluconazole combined with miconazole nitrate（ combined group） and60 cases treated by miconazole nitrate（ control group）,with a 2-week treatment course,and compare clinical effects between the two groups.Results Before the treatment,the serum TNF-ɑ,IL-2,IL-4,IL-6 levels of combined group and control group had no statistical significance（ P〈0. 05）; after treatment,the levels of serum TNF-ɑ,IL-2,IL-4,IL-6 in study group were significantly lower than the control group,and the difference was statistically significant（ P〈0. 05）. After treatment,two groups＇ serum TNF-ɑ,IL-2,IL-4,IL-6 levels were significantly lower than the levels before the treatment,and the difference was statistically significant（ P〈0. 05）.The study group showed an evident effective rate of 80. 00%,effective rate 16. 67% and invalid rate 3. 33%,while the control group showed an evident effective rate of 61. 67%,effective rate 30. 00% and invalid rate 8. 33%,and the difference between the two groups was statistically significant（ P〈0. 05）.There were 4（ 6. 67%） patients in the study group havingadverse reactions,and only 1（ 1. 67%） in the control group had mild nausea,but the incidence of adverse reactions in the two groups was not statistically significant（ P〈0. 05）.Conclusion Fluconazole combined with miconazole nitrate in the treatment of fungal otitis media has better effect and has better anti-inflammatory effect.
