高血压患者脉压与心肺运动功能相关分析 被引量:2

Correlation between pulse pressure and cardiopulmonary exercise capacity in patients with hypertension

摘要 目的探讨高血压患者不同脉压对心肺运动功能的影响。方法选取2016年9月—2017年7月来郑州大学第一附属医院就诊的90例原发性高血压(EH)患者,并按照脉压(PP)进行分组,其中PP<60 mm Hg组46例,PP≥60 mm Hg组为44例,另外选取健康体检者40例(PP<60 mm Hg)为对照组。研究对象均行心肺运动试验(CPET),比较三组CPET相关指标。结果共入选130例患者,EH患者峰值公斤摄氧量(peak VO_2/kg)、峰值VO_2占预计值的百分比(peak VO_2%pred)、峰值摄氧量时的代谢当量(Mets)均低于对照组,差异均有统计学意义(P<0.05),且脉压大组指标的降低较脉压小组差异亦有统计学意义(P<0.05)。经相关分析显示,高血压患者脉压与peak VO_2/kg、peak VO_2%pred均呈负相关。结论高血压患者心肺运动功能受损,且脉压越大,受损程度越重。 Objective To evaluate the correlation between different pulse pressure and cardiopulmonary exercise capacity in patients with essential hypertension. Methods From September 2016 to July 2017,totally 90 cases of patients with essential hypertension( EH) who hospitalized in our center were enrolled. And the patients were divided into two groups according to pulse pressure( PP) : PP 〈 60 mm Hg( n = 46) and PP≥60 mm Hg( n = 44). 40 cases of healthy people( PP 〈 60 mm Hg) were selected as control group. All patients underwent cardiopulmonary exercise test( CPET). Comparison was made among the three groups in CPET indexes. Results There were 130 patients enrolled. Comparing the CPET index of the three groups,we found that peak VO2/kg,peak VO2% pred,peak Mets in all EH patients were significantly reduced than the control group( P 〈 0. 05). And peak VO2/kg,peak VO2% pred,peak Mets were also significantly reduced in the high-PP group of EH patients compared with the low-PP group( P 〈 0. 05). Correlation analysis showed that pulse pressure had a negative correlation to peak VO2/kg、peak VO2% pred. Conclusion Cardiopulmonary exercise capacity was reduced in hypertensive patients and would be aggravated by high pulse pressure.
