消瘀接骨散对骨关节炎患者血瘀指标及NF-κB/RelA通路的影响 被引量:15

Effect of Xiaoyu Jiegu Powder on Blood Stasis Indices and NF-κB/RelA Pathway in Knee Osteoarthritis Patients

摘要 目的观察消瘀接骨散对骨关节炎患者血瘀指标及核因子κB(nuclear factorκB,NF-κB)/相关因子A(related A,RelA)通路的影响。方法选取60例骨关节炎患者,采用随机数字表法分为观察组(消瘀接骨散外敷、氨基葡萄糖口服)及对照组(氨基葡萄糖口服),每组30例,8周后观察两组血瘀指标[D-二聚体(D-dimer,D-D)、纤维蛋白原(fibrinogen,FIB)、血小板活化因子(platelet activating factor,PAF)、血小板活化因子乙水解酶(platelet-activating factor A-hydrolase,PAF-AH)、血栓素B_2(thromboxane B_2,TXB_2)、6-酮-前列腺素F_(1a)(6-keto-prostaglandin F_(1a),6-keto-PGF_(1a))、IL-1、IL-17]、NF-κB/RelA通路相关指标[NF-κB、RelA、转化生长因子β激活性激酶1(transforming growth factor beta-activating kinase,TAK1)、NF-κB衔接蛋白激活剂1(activator1,ACT1)]的变化。结果在治疗期间,观察组脱落2例,对照组脱落1例。与治疗前比较,治疗8周后,观察组D-D、FIB、PAF、TXB_2均明显降低,两组PAF-AH、6-keto-PGF_(1a)均明显升高,IL-1、IL-17、NF-κB、RelA、TKA1、ACT1均明显降低(P<0.05,P<0.01)。与对照组治疗后比较,观察组FIB、TXB_2降低,6-ketoPGF_(1a)升高更明显,IL-17、NF-κB、RelA降低更明显(P<0.05,P<0.01)。结论消瘀接骨散通过抑制NF-κB/RelA通路激活,下调IL-1、IL-17细胞因子表达,改善骨关节炎患者关节症状及血瘀状态。 Objective To observe the effect of Xiaoyu Jiegu Powder( XYJGP) on blood stasis indices and nuclear factor κB( NF-κB)/related A( RelA) pathway in patients with knee osteoarthritis( KOA). Methods Sixty KOA patients were assigned to the observation group( oral administration of XYJGP and glucosamine) and the control group( oral administration of glucosamine) by random digit table,30 cases in each group. After 8 weeks,changes of blood stasis indicess such as D-dimer( DD),fibrinogen( FIB),platelet activating factor( PAF),platelet-activating factor A-hydrolase( PAF-AH),thromboxane B2( TXB2),6-keto-prostaglandin F1a( 6-keto-PGF1a),IL1,IL-17,NF-κB/RelA pathway related factors [NF-κB,RelA,transforming growth factor-β activator kinase 1( TAK1),NF-κB adapter activator 1 ACT1) ] were observed in the two groups. Results During the treatment period,2 cases in the observation group and 1 case in the control group dropped out. After 8 weeks of treatment,the levels of DD,FIB,PAF,and TXB2 significantly decreased in the observationgroup,as compared with those of the control group. Levels of PAF-AH and 6-keto-PGF1awere significantly increased,levels of IL-1,IL-17,NF-κB,RelA,TKA1,and ACT1 were significantly decreased in the two groups(P 〈 0. 05,P 〈 0. 01).Compared with the control group after treatment,levels of FIB and TXB2 decreased significantly,6-keto-PGF1aincreased more significantly,levels of IL-17,NF-κB,and RelA decreased more significantly in the observation group( P 〈 0. 05,P 〈0. 01). Conclusion XYJGP could improve joint symptoms and blood stasis state of KOA patients by inhibiting the activation of NF-κB/RelA pathway and down-regulating the expressions of IL-1 and IL-17.
