摘要
目的:探讨外周血中性粒细胞与淋巴细胞比值在2型糖尿病合并急性脑梗死患者中的临床意义,为临床诊治提供理论依据。方法:选取本院神经内科2014年1月~2016年12月就诊的2型糖尿病合并急性脑梗死患者136例作为研究组,同期体检的68例糖尿病患者作为对照组,检测患者白细胞计数(WBC)、中性粒细胞计数(NC)、淋巴细胞计数(LC)及中性粒细胞/淋巴细胞比值(NLR)。通过受试者工作特征曲线(ROC)和曲线下面积(AUC)评价WBC、NC、LC、NLR在2型糖尿病合并急性脑梗死患者诊断中的作用。结果:研究组患者WBC、NC、NLR的水平明显高于对照组(P<0.05);而LC水平明显低于对照组(P<0.05)。研究组患者WBC、NC、NLR的水平随着NIHSS评分升高而增加(P<0.05);而LC水平随着NIHSS评分升高而降低(P<0.05)。NLR、WBC、NC及LC诊断糖尿病合并急性脑梗死曲线下面积分别为0.856、0.717、0.685、0.321,灵敏度分别为0.838、0.596、0.517、0.208,特异度分别为0.868、0.838、0.793、0.559。结论:NLR可以作为诊断2型糖尿病合并急性脑梗死患者的生物标志物。
Objective:To investigate the clinical significance of peripheral blood neutrophil to lymphocyte ratio in patients with2 type diabetes mellitus complicated acute cerebral infarction,so as to provide theoretical basis for clinical diagnosis and treatment.Methods:From Jan.2014 to Dec.2016 year,136 patients with 2 type diabetes mellitus complicated acute cerebral infarction were selected as research group and 68 diabetic patients undergoing physical examination were selected as the control group.White blood cell count(WBC),neutrophil count(NC),lymphocyte count(LC) and neutrophil to lymphocyte ratio(NLR) were detected and calculated.The role of WBC,NC,LC and NLR in the diagnosis of type 2 diabetes mellitus combined with acute cerebral infarction were evaluated by receiver operating characteristic curve(ROC) and area under the curve(AUC).Results:The levels of WBC,NC and NLR in research group were significantly higher than those of in control group(P〈0.05),while the level of LC was significantly lower than that of in control group(P〈0.05).The levels of WBC,NC and NLR in research group increased with the increase of NIHSS score(P〈0.05),and the level of LC decreased with the increase of NIHSS score(P〈0.05).The area under the curve of NLR,WBC,NC and LC in diagnosing diabetes combined with acute cerebral infarction were respectively 0.856,0.717,0.685 and 0.321.The sensitivity were respectively 0.838,0.596,0.517 and 0.208.The specificity are respectively 0.868,0.838,0.793 and 0.559.Conclusion:NLR could be used as a marker for the diagnosis of type 2 diabetes with acute cerebral infarction.
