摘要
目的分析经椎板间入路椎间孔镜治疗腰椎间盘突出症的近期临床疗效,对比中青年与老年患者的近期临床疗效差异。方法回顾性分析2014年10月—2017年3月因腰椎间盘突出症在该院行经椎板间入路椎间孔镜手术治疗并取得6个月以上随访患者155例,按年龄60岁以上为老年组,60岁以下为中青年组,对比两组患者术前、术后1 d、1个月、6个月VAS评分、ODI指数,末次随访增加Mac Nab改良评价。结果两组患者术后VAS评分、ODI指数较术前均明显改善;术后1 d、6个月中青年组VAS评分[(2.27±0.927)分、(0.38±0.504)分]、ODI指数[(23.07±7.670)分、(1.48±0.913)分]结果优于老年组[(2.29±0.624)分、(1.04±0.706)分、(22.25±5.840)分、(3.65±2.707)分],末次随访中青年组Mac Nab评价满意度(93.1%)高于老年组(70.8%)。结论因腰椎间盘突出症经椎板间入路椎间孔镜治疗的患者近期临床疗效令人满意,且中青年患者优于老年患者。
Objective To analyze the clinical curative effect of interlaminar intervertebral endoscopy in treatment of middle-aged, young and senile prolapse of lumbar intervertebral disc. Methods 155 cases of patients undergoing the interlaminar intervertebral endoscopy and followed up for more than six months from October 2014 to March 2017 were selected and divided into two groups, the senile group were the patients aged more than 60 years old, while the middleaged and youth group were the patients below 60 years old, and the VAS scores, ODI index, Mac Nab modified evaluation before surgery, in 1 d, 1 month and 6 months were compared between the two groups. Results The VAS score and ODI index of the two groups after surgery were obviously improved compared with those before surgery, and the VAS scores and ODI index in 1 d and 6 months after surgery of the two groups were better than those in the senile group,[（2.27 ±0.927）points,（0.38 ±0.504）points,（23.07 ±7.670）points,（1.48 ±0.913）points vs（2.29 ±0.624）points,（1.04 ±0.706）points,（22.25±5.840）points,（3.65±2.707）points], and the Mac Nab evaluation satisfactory degree in the youth group in the last follow-up was higher than that in the senile group,（93.1% vs 70.8%）. Conclusion The clinical curative effect of interlaminar intervertebral endoscopy in treatment of middle-aged, young and senile prolapse of lumbar intervertebral disc is satisfactory, and the clinical curative effect in the middle-aged and youth group is better than that in the senile patients.
