摘要
目的探讨N-乙酰半胱氨酸(N-acetylcysteine,NAC)对肺纤维化合并肺气肿综合征(combined pulmonary fibrosis and emphysema syndrome,CPFE)患者血清炎症细胞因子的影响。方法将2013年6月至2016年6月在重庆市人民医院呼吸内科门诊及住院的CPFE患者112例按照入组先后顺序排序,单号为治疗组,双号为对照组,每组各56例。两组都给予西医常规抗炎、支气管扩张剂及氧疗等基本治疗,治疗组除了给予基本治疗,还给予口服NAC治疗12个月。于治疗前,治疗3、6、9、12个月分别抽取受试者早晨空腹静脉血并处理,留取标本,运用酶联免疫法对各标本中的IL-8、TNF-α、TGF-β1、VEGF水平进行定量检测。结果从NAC治疗6个月后,治疗组与对照组比较IL-8、TNF-α、TGF-β1水平明显下降(P<0.001),VEGF水平明显升高(P<0.001)。且随着治疗时间的推移,后一时间点细胞因子水平的改善比前一时间点更显著。结论 NAC治疗CPFE综合征时,随着时间的推移可以显著改善IL-8、TNF-α、TGF-β1、VEGF等细胞因子水平的表达。
Objective To explore the effect of N-acetylcysteine （NAC） on serum inflammatory cytokines in the patients with combined pulmonary fibrosis and emphysema syndrome （CPFE）. Methods From June 2013 to June 2016, 112 CPFE outpatient and hospitalized patients admitted in the respiratory department were prospectively recruited in this study, and according to the order of entry, these with odd number were assigned into the treatment group, while those of double number into the control group, with 56 cases in each group. Both groups were given anti-inflammatory agents, bronehodilator and oxygen therapy and other basic therapy, while the patients of the treatment group were given oral NAC treatment for 12 months additionally. Morning fasting venous blood samples were harvested before treatment and in 3, 6, 9 and 12 months after treatment, and the levels of IL-8, TNF-α, TGF-β1 and VEGF were detected with enzyme-linked immunoassay. Results Compared with the control group, the levels of IL-8, TNF-α and TGF-β1 were decreased significantly （P 〈 0. 001 ）, and that of VEGF was increased obviously （P 〈 0. 001 ） in 6 months after treatment. Moreover, with the elapse of time, more significant improvement was observed in the cytokine levels than those at the previous time points. Conclusion In the treatment of CPFE syndrome, NAC can significantly improve the expression of cytokines, such as IL-8, TNF-α, TGF- β1 and VEGF as time goes on.
