摘要
目的了解泸州市基层医务人员职业倦怠与社会支持的关系。方法采取分层整群随机抽样的方法对泸州市749名乡镇卫生院基层医务人员进行自填式问卷调查。结果调查对象职业倦怠的重度情绪衰竭者占18.7%,重度情绪淡漠者占18.8%,重度低个人成就感者占64.7%。多因素分析显示,调整年龄、文化程度、岗位、工龄、每天工作时间、健康状况及有无子女因素后,客观支持与重度情绪衰竭呈负相关(OR=0.91,95%CI为0.84-0.98);调整性别、工龄因素后,客观支持和主观支持与重度情感淡漠均呈负相关(OR分别为0.89、0.74);调整年龄、文化程度、岗位、工龄、月收入、有无子女因素后,客观支持与重度低个人成就感呈负相关(OR=0.86,95%CI为0.81-0.92)。结论泸州市基层医务人员的职业倦怠处于较高水平,社会支持程度尤其是客观支持与职业倦怠有一定关系,拥有较高的社会支持能够使医务人员保持良好的工作积极性。
Objective To investigate the relationship between occupational burnout and social support in primary medical staff. Methods 749 primary medical staff in Luzhou were invested by self-filling questionnaires through stratified cluster random sampling. Workers with occupational noise exposure were chosen as noise group, and people with no occupational noise exposure and randomly took health examination in hospital as control group. The noise group were divided into non smoking group, smoking group and passive smoking group. hearing results and cumulative noise exposure(CNE) was compared among these groups. Result the dimensions of emotional exhaustion, the prevalence of severe emotional indifference was 18.7%,severe emotional detachment 18.8 %,and severe low personal achievement 64.7 %. Multiple factors analysis showed that after adjusting the factors of age,cultural level,position,length of service,working time each day,health status,and presence of children,objective support was negatively correlated with severe emotional exhaustion(OR=0.91,95%CI is 0.84-0.98). After adjusting the factors of gender and the length of service,objective support and subjective support were negatively correlated with severe emotional indifference(OR is 0.89 and 0.74 respectively). Adjusted age,culture level, position, length of service, monthly income, and presence of children, objective support was negatively correlated with severe low personal achievement(OR=0.86,95%CI is 0.81-0.92). Conclusion The occupational burnout in primary medical staff of Luzhou is at a high level. The social support especially objective support have certain relations with occupational burnout. With high social support,medical staff can maintain good working enthusiasm.
