减少老年患者出院过渡期药物不良事件的护理研究现状 被引量：1

摘要 药物不良事件（adverse drug event,ADE）,是指与药物使用有关的意外和有害事件^（[1]）。在美国,每年因严重ADE增加220万次住院、850亿美元的医疗费用,以及造成10.6万人死亡^（[2]）。ADE的类型为用药错误、药物不良反应、治疗失败和戒断反应^（[3]）。过渡期护理模式是指患者在疾病诊疗和康复阶段,护士为确保过渡期间护理工作的协调与连续而采取的一系列相应的护理行为。
