摘要
目的:研究优质护理对老年慢阻肺患者的肺功能和生活质量的影响分析。方法:选取2014年10月-2017年2月收治的102例慢阻肺患者,按照护理方法不同划分为两组,包括干预组(优质护理干预)与常规组(一般护理),各51例,评价两组患者生活质量评分、护理质量评分及护理干预前后肺功能指标值,其中肺功能指标包括PaO_2(血氧分压)、FEV_1(1 s用力呼气容积)、PaCO_2(动脉血二氧化碳分压)等。结果:干预组患者对护士提供的生活指导、呼吸锻炼、饮食指导、健康宣教等护理质量评分均高于常规组,差异均有统计学意义(t=33.759、58.342、36.345、38.306,P<0.05)。干预组患者运动、健康状态、活动、生活行为等生活质量评分均高于常规组,差异均有统计学意义(t=39.250、43.953、42.234、28.758,P<0.05)。干预前,两组患者PaO_2、FEV_1、PaCO_2等肺功能指标比较差异无统计学意义(t=1.211、0.621、0.058,P>0.05);护理干预后,干预组患者PaO_2、FEV_1、PaCO_2等肺功能指标值结果均优于干预组,差异均有统计学意义(t=9.546、42.975、11.125,P<0.05)。结论:为老年慢阻肺患者提供优质护理干预,可提高生活质量及护理质量,改善患者肺功能。
Objective：To analyze the clinical effect of high quality nursing interventions in the pulmonary function and life quality for elderly patients with COPD（chronic obstructive pulmonary disease）.Method：A total of 102 elderly COPD patients treated from October 2014 to February 2017 in the author＇s hospital were selected.The subjects were divided into two groups according to the different nursing methods,51 cases each group.The intervention group received the high quality nursing interventions,the conventional group received the conventional nursing care.The life quality,nursing quality and pulmonary function index were evaluated.Result：The nursing scores of living guidance,breathing exercises,dieting guidance and health education in intervention group were higher than conventional group,the differences were statistically significant（t=33.759,58.342,36.345,38.306,P〈0.05）.The evaluation scores of movement,health status,daily activity and life conducts for intervention group were higher than conventional group,the differences were statistically significant（t=39.250,43.953,42.234,28.758,P〈0.05）.Before intervention, the pulmonary function index of PaO_2,FEV_1 and PaCO_2 for two groups were not significantly different（t=1.211,0.621,0.058,P〉0.05）,after intervention,the index of PaO_2,FEV_1 and PaCO_2 in intervention group were significantly better than conventional group,the differences were statistically significant（t=9.546,42.975,11.125,P〈0.05）.Conclusion：For elderly COPD patients, the high quality nursing interventions can improve the life quality,nursing quality and pulmonary function.
