摘要
目的评价比阿培南治疗儿童重症社区获得性肺炎(CAP)的效果及安全性。方法以2016年1—11月间入住我院的50例重症CAP病儿为研究对象,给予比阿培南10mg/kg每12h静脉滴注1次,治疗时间为10~14d。检测并比较治疗前后病儿外周血白细胞计数(WBC)、血清C反应蛋白(CRP)、降钙素原(PCT)、动脉血氧分压(PaO2)、血清谷丙转氨酶(ALT)、谷草转氨酶(AST)、尿素氮(BUN)、肌酐(Cr)的变化,并进行临床疗效及细菌学疗效评价;观察比阿培南治疗的不良反应。结果总有效率为92%,其中痊愈39例,显效7例,进步3例,无效1例。病儿治疗后WBC、CRP、PCT均明显低于治疗前(t=3.52~4.07,P<0.05);PaO2明显高于治疗前(t=4.19,P<0.05);血清ALT、AST、BUN、Cr水平治疗前后比较差异无显著性(P>0.05)。24例痰液病原体检测阳性病儿的痰液细菌清除率为95.8%。所有病儿均未观察到严重不良反应。结论比阿培南治疗重症CAP病儿有良好的效果及较高的安全性。
Objective To evaluate the clinical efficacy and safety of biapenem in the treatment of children with severe community-acquired pneumonia（CAP）. Methods A total of 50 children with severe CAP who were admitted to our hospital from January to November,2016 were taken as research subjects,and were given intravenous infusion of biapenem 10 mg/kg once every 12 hfor 10-14 d.Peripheral white blood cell count（WBC）,serum C-reactive protein（CRP）,procalcitonin（PCT）,arterial partial pressure of oxygen（PaO2）,alanine aminotransferase（ALT）,aspartate aminotransferase（AST）,blood urea nitrogen（BUN）,and creatinine（Cr）of patients were determined and compared before and after the treatment;clinical efficacy and bacteriological efficacy were evaluated;adverse reactions to biapenem treatment was observed. Results The overall response rate was92%,including 39 cases cured,7 cases showing marked response,3 cases improved,and 1 case with no response.After treatment,the patients showed significant reductions in WBC count,CRP,and PTC（t=3.52-4.07,P〈0.05）,a significant increase in PaO2（t=4.19,P〈0.05）,and no significant changes in ALT,AST,BUN,and Cr levels（P〉0.05）.The sputum bacterial clearance rate was 95.8%in 24 sputum pathogen-positive patients.No severe adverse reactions were observed in all 50 patients with severe CAP. Conclusion Biapenem has good efficacy and high safety in the treatment of children with severe CAP.
