摘要
目的研究支气管扩张伴感染患者细菌构成及造成的直接经济损失。方法回顾性选取我院近两年支气管扩张伴感染患者敏感细菌感染和多重耐药菌感染住院患者各78例,比较两组细菌构成、住院费用和患者住院日。结果敏感细菌感染组前三位分别为铜绿假单胞菌(70.5%)、肺炎克雷伯菌(10.3%)和鲍曼不动杆菌(5.1%);多重耐药菌感染组前三位分别为铜绿假单胞菌(47.4%)、大肠埃希氏菌(20.5%)和鲍曼不动杆菌(15.4%),两组细菌构成存在差异(P<0.05)。敏感细菌感染组和多重耐药菌感染组住院日分别为(10.9±2.5)天和(13.8±5.5)天,住院费用分别为(7636.1±2671.4)元和(11601.2±5579.1)元,差异均有统计学意义(P<0.05)。结论支气管扩张伴感染患者敏感细菌感染和多重耐药菌感染细菌构成不同,且多重耐药菌感染导致医院和患者严重的经济负担,浪费卫生资源。因此,预防细菌耐药意义重大,需多措并举。
Objective To study the bacterial composition and direct economic burden in patients with bronchiectasis and infection. Methods A total of 156 patients with bronchiectasis and infection were divided into two groups,with 78 cases in each group. The distribution of pathogenic bacteria and direct economic loss were analyzed.Results The first three germs were Pseudomonas aeruginosa（ 70. 5%）,Klebsiella pneumoniae（ 10. 3%） and Bauman Acinetobacter（ 5. 1%） in the sensitive bacterial infection group,and Pseudomonas aeruginosa（ 47. 4%）,Escherichia coli（ 20. 5%） and Bauman Acinetobacter（ 15. 4%） in the multi-drug resistant bacteria infection group（ P〈0. 05）. The average duration of hospital stay was（ 10. 9 ± 2. 5） days and（ 13. 8 ± 5. 5） days respectively in the sensitive bacterial infection group and the multi-drug resistant bacteria infection group,and the average of hospital charge was（ 7636. 1 ± 2671. 4） Yuan and（ 11601. 2 ± 5579. 1） Yuan respectively（ P〈0. 05）. Conclusion The composition ratio of sensitive and multi-drug resistant bacteria is different in patients with bronchiectasis and infection. Multi-drug resistant bacteria infection causes severe economic burden for hospital and patients,wasting healthy resources. Therefore,the prevention of bacterial resistance is of great significance,and many measures should be carried out.
