摘要
目的:探讨带状疱疹(HZ)患者血浆S100β蛋白、神经元特异性烯醇化酶(NSE)、免疫球蛋白含量及水痘-带状疱疹病毒(VZV)载量与病情严重程度的相关性。方法:选取2015年1月～2017年1月在本院确诊的HZ患者89例为研究对象,另选取同期体检的健康者30名为正常对照。采用实时荧光定量PCR(RT-PCR)检测疱液中VZV载量水平,采用ELISA法检测血浆S100β、NSE水平,采用免疫比浊法检测免疫球蛋白(Ig G、Ig M、Ig A)含量,对治疗前病情严重程度进行评分,并记录患者随访期间带状疱疹后遗神经痛(PHN)发生情况。结果:HZ患者血浆S100β、NSE水平明显高于健康对照组(P<0.05),患者疱液中VZV DNA载量、S100β、NSE水平与HZ病情严重程度评分呈正相关(r=0.822、0.756、0.689,P均<0.05);HZ患者血浆Ig G、Ig M、Ig A与对照组无显著差异(P>0.05),且与病情严重程度评分无明显相关性(r=-0.132、0.244、0.179,P均>0.05),但其在发生PHN和未发生PHN组间的差异有统计学意义(P<0.05)。结论:HZ患者血浆S100β、NSE水平及VZV DNA载量均与病情严重程度密切相关,在病情评估中具有重要临床意义。
Objective： To investigate the correlation between the levels of S100β,neuron-specific enolase（ NSE）,immunoglobulin,varicella-zoster virus（ VZV） DNA and severity score in patients with herpes zoster（ HZ）. Methods： A total of 89 HZ patients diagnosed in our hospital from January 2015 to January 2017 were selected as the subjects,and 30 healthy subjects were selected as the normal control. The levels of VZV in blister fluid were detected by real-time fluorescence quantitative PCR（ RT-PCR）,the levels of S100βand NSE in plasma were detected by enzyme-linked immunosorbent assay（ ELISA）,and the levels of immunoglobulin（ Ig G,Ig M,Ig A）were measured by immunoturbidimetry. The severity of the disease before treatment was scored,and the incidence of postherpetic neuralgia（ PHN） during the follow-up period was recorded. Results： The levels of S100β and NSE in HZ patients were significantly higher than those in healthy subjects（ P〈0. 05）,and the VZV DNA load,S100β and NSE levels in patients with HZ were positively correlated with the severity of HZ（ r = 0. 822,0. 756,0. 689,P〈0. 05）. There was no significant difference in serum Ig G,Ig M and Ig A between HZ patients and healthy subjects（ P〉 0. 05）,and there was no significant correlation with the severity of HZ patients（ r =-0. 132,0. 244,0. 179,P〉 0. 05）,but the difference in Ig G,Ig M and Ig A levels between patients with PHN and without PHN was statistically significant（ P〈 0. 05）. Conclusion： The levels of plasma S100β,NSE and VZV DNA in HZ patients are closely related to the severity of HZ,and they have important clinical significance in the assessment of the disease.
