摘要
目的:探讨克罗恩病的临床病理特点。方法:所选研究对象为某院收治的克罗恩病患者,年限为2015年2月~2017年2月,符合标准的有23例,患者及家属均对本研究知情同意。回顾性分析23例患者的临床资料,并对其临床病理特点进行分析。结果:在病变部位上,14例为回肠结肠,5例为小肠,4例为结肠;在临床表现上,21例患者出现腹痛,17例患者出现腹泻,10例患者出现贫血,20例大便隐血试验阳性,其他临床表现不突出,且不具特异性,多数患者合并2种或以上表现;在并发症上,10例肠梗阻,3例肠穿孔,4例肠瘘,2例肛周溃疡,1例口腔溃疡,1例结膜炎,1例腹腔脓肿,1例胆囊炎;在内镜特点上,病变呈节段性分布13例,存在溃疡性病变21例,5例肠腔狭窄,7例鹅卵石样变,3例结节状增生,不同形态溃疡可重叠出现;在病理改变上,手术标本6例,活检标本20例,手术标本中非干酪样肉芽肿发生率较高,活检标本中发现率较低,对比有统计学意义(P<0.05)。结论:克罗恩病临床表现多样,诊断中肠镜检查发挥着重要的作用,而内镜与病理活检联合应用,能提升诊断准确性。
Objective：To explore the clinicopathological features of crohn＇s disease.Methods：The selecteded subjects were crohn＇s patients who were admitted to a hospital from February 2015to February 2017,and 23 cases were eligible.Both patients and their families informed consent of the study.Clinical data of 23 patients were analyzed retrospectively and their clinicopathological features were analyzed.Results：In the lesion site,14cases were ileum,5were small intestine and 4were colon.On the clinical manifestations,21 cases of patients with abdominal pain,17patients appeared diarrhea,anemia 10patients,20cases of defecate occult blood test was positive,other clinical performance was not outstanding,and nonspecific,most patients present merge two or more.In complications,10cases of intestinal obstruction,3intestinal perforations,4cases of intestinal fistula,2cases of perianal ulcer,1case of oral ulcer,1case conjunctivitis,1case of abdominal abscess,1case cholecystitis.On the endoscopic features,pathological changes in 13cases with segmental distribution,existence ulcer lesions 21cases,5cases of stenosis lumen,7cases of pebbles sample,3cases of nodular hyperplasia,ulcer may overlap in different form.On the pathological changes,surgical specimens of 6cases,biopsy specimens of 20cases,surgical specimens were higher in non-caseous granuloma,biopsy specimens discovery ratewas low,the contrast was statistically significant（P〈0.05）.Conclusion：The clinical manifestations of crohn＇s disease are diverse,and the diagnosis of colonoscopy plays an important role,and the combination of endoscopy and pathological biopsy can improve the diagnostic accuracy.
