摘要
目的对成人隐匿性自身免疫性糖尿病(LADA)的氧化应激状态进行评估,为临床决策提供依据。方法收集70例LADA患者、100例2型糖尿病(T2DM)、100例健康体检者的血标本及临床随访资料,检测甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、糖化血红蛋白(HbA1_C)及氧化应激指标异前列腺素F2α(8-iso PGF2α)和抗氧化指标超氧化物歧化酶(SOD)。结果 LADA组的HDL-C高于T2DM组,而LDL-C和TG低于T2DM组,差异具有统计学意义(P<0.05)。LADA组的8-iso PGF2α低于T2DM组而高于体检组,而SOD高于T2DM组但低于体检组,差异具有统计学意义(P<0.05)。LADA组8-iso PGF2α与HbA1_C、LDL-C及病程呈正相关;SOD与HbA1_C呈负相关,与HDL-C呈正相关。LADA患者随访半年时HbA1_C和8-iso PGF2α较住院时显著下降,差异具有统计学意义(P<0.05)。结论 LADA患者具有独特的氧化应激及抗氧化机制,短期血糖控制可以改善氧化应激,但对抗氧化无显著影响。
Objective To evaluate the oxidative stress status of latent autoimmune diabetes in adults (LADA) and provide a support for clinical decision-making. Methods The blood specimens and clinical follow-up data from 70 patients with LADA, 100 cases of type 2 diabetes mellitus (T2DM) and 100 cases of healthy physical examination were collected. The level of triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), glycosylated hemoglobin (HbA1C), isoprostaglandins F2α (8-isoPGF2α) and superoxide dismutase (SOD) were detected. Results HDL-C in LADA group were higher than in T2DM group, while LDL-C and TG in LADA group were lower than in T2DM group , the difference was statistically significant (P 〈 0.05). 8-isoPGF2α in LADA group was lower than in T2DM group and higher than in normal control group, while SOD in LADA group was higher than in T2DM group but lower than in normal control group, the diference was statistically significant (P 〈 0.05). The 8-isoPGF2α in LADA group was positively correlated with HbA1C, LDL-C and disease course. The SOD was negatively correlated with HbA1C and positively correlated with HDL-C. In LADA patients, HbA1C and 8-isoPGF2α were signifcantly decreased after six months of follow-up compared with those in hospital, the difference was statistically signifcant (P 〈 0.05). Conclusion LADA patients have an unique mechanism of oxidative stress and anti-oxidation, and short-term blood glucose control can improve oxidative stress, but it has no signifcant efect on anti-oxidation.
