摘要
心肌纤维化是多种心血管疾病发展至一定阶段的共同病理改变,符合血瘀证的微观诊断。文章从中西医角度分析"瘀毒"病机特点,依据"心血管事件链"规律,结合"瘀毒"与心力衰竭基础病的关系,探讨其与心力衰竭心肌纤维化的密切关系。慢性心血管疾病日久不愈、诸邪久积不解、蕴毒骤发,"瘀毒互结,损伤血络",引起心脏结构和功能改变是心力衰竭心肌纤维化的病理基础;"瘀毒既成,百证丛化,正气亏虚,变生诸病"是其突发骤变、错综复杂、缠绵难愈的关键环节。心肌纤维化以瘀毒为害,初期虽未彰显,有可寻察之外证时往往病属晚期,因其持续进展过程和不良结局,未病先防,治疗关口前移显得尤为重要。因此,活血解毒应成为贯穿心肌纤维化治疗过程的重要方法之一。
Myocardial fibrosis is one of the main pathological changes in the development of various cardiovascular diseases, which is in line with the microscopic diagnosis of blood stasis syndrome. This article analyzed the pathogenesis characteristics of 'stasis-toxin' from the perspectives of traditional Chinese medicine and western medicine, according to the law of 'cardiovascular event chain' and combining 'stasis-toxin' with the basic diseases of heart failure, to explore the close relationship with 'stasis-toxin' and myocardial fibrosis of heart failure. Chronic cardiovascular diseases were not cured for a long time, various pathogenic factors accumulate for a long time, and there was a sudden occurrence of toxin. The pathologic basis of myocardial fibrosis of heart failure is 'stasis and toxin interlock, damage blood collaterals', which causes changes in heart structure and function. 'Blood stasis and toxin are formed, all the syndromes condense, deficiency of positive qi, and transform into various diseases' are the key links for sudden changes, intricacies, and lingering difficulties. Myocardial fibrosis is caused by stasis-toxin. Although it is not obvious in the early stage, it is often advanced when there is evidence to be found. Because of its continuous progress and adverse outcomes, it is particularly important to prevent the disease from moving forward. Therefore, promoting blood circulation and detoxication should be one of the important methods for the treatment of myocardial fibrosis.
