摘要
目的系统评价抚触对早产儿早期的干预效果。方法2017年5月,计算机检索PubMed、Cochrane Library、EMBASE、Web of Science、中国生物医学文献数据库(Chinese Biology Medicine disc,CBMdisc)、中国期刊全文数据库(Chinese National Knowledge Infrastructure,CNKI)、万方数据库有关抚触干预对早产儿临床效果的随机对照试验(randomized controlled trial,RCT)。结局指标包括体质量、身长、头围、摄奶量、呼吸系统并发症、胃潴留、腹胀、血胃动素等。检索时间为建库至2017年5月8号。采用Cochrane系统评价手册5.0推荐的偏倚风险评估工具对纳入RCT进行方法学质量评价,使用RevMan 5.3软件对资料进行Meta分析。结果共纳入15个RCT,1114例早产儿。Meta分析结果显示:与对照组相比,抚触可增加早产儿体质量、身长、头围、摄奶量和血胃动素水平,并降低早产儿胃潴留、腹胀及呼吸系统疾病等不良事件的发生率,差异均有统计学意义(均P<0.05)。结论抚触可促进早产儿早期生长发育,并有降低胃肠功能紊乱及呼吸系统疾病发生的可能。
Objective To evaluate the intervention effect of massage therapy in preterm infants.Methods The randomized controlled trials （RCTs） about massage therapy in preterm infants were searched in PubMed, Cochrane Library, EMbase, Web of Science databases,China Biology Medicine disc （CBM）,China National Knowledge Infrastructure （CNKI）,and Wanfang Data from inception to May 2017.Outcome measures included weight, length, head circumference,milk intake, respiratory complications, gastric retention, abdominal distention and motilin.The RevMan5.3 software was used for a Meta-analysis.Results A total of 15 RCTs involving 1114 preterm infants were eligible for inclusion in the Meta-analysis. The result of meta-analysis showed that the massage therapy increased the weight of infants,the length of infants,the head circumference of infants,the intake of milk,and the blood content of motilin.Compared to the routine therapy group, the massage therapy group can lower incidences of gastric retention,abdominal distension,and respiratory tract disease at early stage of preterm infants （all P 〈0.05 ）.Conclusions Massage therapy can promote early growth and development of premature infants, and reduce their gastrointestinal disorders and respiratory diseases.
