摘要
目的探讨不同病理分型、症状严重程度慢性前列腺炎(chronic prostatitis,CP)患者前列腺液免疫球蛋白(immunoglobulin,Ig)、炎性因子、趋化因子水平变化及临床意义。方法 148例CP患者,依据美国国立卫生研究院病理分型分为Ⅱ型组48例、ⅢA型组56例、ⅢB型组44例,依据国际慢性前列腺炎症状指数分为轻度组49例、中度组62例、重度组37例;同期45例男性体检健康者为对照组。检测各组前列腺液IgG、IgA、IgM、白细胞介素(interleukin,IL)-6、IL-8、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、巨噬细胞炎性蛋白-1α(macrophage inflammatory protein-1α,MIP-1α)、单核细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)水平,并进行比较。结果Ⅱ型组、ⅢA型组、ⅢB型组前列腺液IgG[(8.35±2.11)、(9.26±2.70)、(8.95±2.50)g/L]、IgA[(2.95±0.37)、(2.88±0.34)、(3.11±0.43)g/L]、IgM[(2.83±0.21)、(2.72±0.46)、(2.92±0.48)g/L]、IL-6[(0.32±0.17)、(0.36±0.16)、(0.23±0.06)μg/L]、IL-8[(0.37±0.17)、(0.34±0.14)、(0.21±0.13)μg/L]、TNF-α[(0.70±0.26)、(0.71±0.26)、(0.55±0.15)μg/L]、MIP-1α[(215.35±20.48)、(181.26±16.89)、(155.76±14.62)ng/L]、MCP-1[(352.66±33.67)、(287.68±29.74)、(240.47±32.68)ng/L]水平均高于对照组[(2.35±0.27)g/L、(0.49±0.17)g/L、(0.34±0.13)g/L、(0.14±0.08)μg/L、(0.17±0.02)μg/L、(0.44±0.15)μg/L、(131.3±10.95)ng/L、(202.43±21.53)ng/L](P<0.05);Ⅱ型、ⅢA型、ⅢB型组前列腺液IgG、IgA、IgM水平比较差异无统计学意义(P>0.05);Ⅱ型、ⅢA型组前列腺液IL-6、IL-8、TNF-α、MIP-1α、MCP-1水平高于ⅢB型组(P<0.05);Ⅱ型组前列腺液MIP-1α、MCP-1水平高于ⅢA型组(P<0.05),IL-6、IL-8、TNF-α水平与ⅢA型组比较差异无统计学意义(P>0.05);轻、中、重度组前列腺液IgG[(7.06±1.21)、(10.31±1.86)、(15.81±3.06)g/L]、IgA[(2.37±0.73)、(3.92±1.04)、(5.89±1.30)g/L]、IgM[(2.48±0.62)、(3.57±0.90)、(4.97±1.11)g/L]、IL-6[(0.28±0.09)、(0.36±0.09)、(0.45±0.11)μg/L]、IL-8[(0.32±0.06)、(0.39±0.16)、(0.51±0.18)μg/L]、TNF-α[(0.60±0.09)、(0.67±0.26)、(0.92±0.34)μg/L]、MIP-1α[(149.25±13.89)、(172.66±18.68)、(248.98±21.15)ng/L]、MCP-1[(241.06±20.83)、(288.61±27.32)、(379.55±32.66)ng/L]水平均高于对照组(P<0.05),且中、重度组高于轻度组,重度组高于中度组(P<0.05)。结论 CP患者前列腺液免疫球蛋白IgG、IgA、IgM,炎性因子IL-6、IL-8、TNF-α,趋化因子MIP-1α、MCP-1水平明显增高,且不同病理分型、症状严重程度CP患者表达存在差异,检测各指标水平有助于病理分型、症状严重程度的辅助诊断。
Objective To investigate the changes of immunoglobulin （Ig）, inflammatory factors and chemokines in prostatic fluid of patients with chronic prostatitis （CP） and their clinical signit：icances. Methods Totally 148 patients with CP were divided type Ⅱ group （n= 48）, type ⅢA group （n= 56） and type Ⅲ B group （n= 44） according to the pathological classification proposed by the National Institute of Health, and were divided into mild group （n = 49）, moderate group （n= 62） and severe group （n= 37） according to International Chronic Prostatitis Symptom Indexes. Another 45 healthy volunteers were as controls （control group）. The levels of IgG, IgA, IgM, interleukin （IL）-6, IL-8, tumor necrosis factor-α （TNF-α）, macrophage inflammatory protein-1α （MIP-1α） and monocyte chemoattractant protein-1 （MCP-1） were detected and compared among groups. Results The levels of IgG （（8.35 ± 2. 11 ）, （9.26 ± 2.70）, （8.95±2.50） g/L）, IgA （（2.95±0.37）, （2.88±0.34）, （3.11±0.43） g/L）, IgM （（2.83±0.21）, （2.72± 0.46）, （2. 92±0.48） g/L）, IL-6 （（0. 32±0.17）, （0. 36±0.16）, （0. 23±0.06） μg/L）, IL-8 （（0. 37±0.17）, （0. 34± 0.14）, （0.21±0.13） μg/L）, TNF-α（（0. 70±0. 26）, （0. 71±0. 26）, （0. 55±0. 15） μg/L）, MIP-1α（（215. 35± 20.48）, （181. 26±16.89）, （155.76 ± 14. 62） ng/L） and MCP-1 （（352.66±33.67）, （287. 68±29. 74）, （240.47± 32.68） ng/L） in type Ⅱ, Ⅲ A, and ⅢB groups were significantly higher than those in control group （（2. 35 ±0.27） g/L, （0.49±0.17）g/L, （0.34±0.13）g/L, （0.14±0.08） μg/L, （0. 17±0.02） μg/L, （0.44±0. 15） μg/L, （131.3±10.95） ng/L, （202.43±21.53） ng/L） （P〈0.05）. There were no significant differences in the levels of IgG, IgA and IgM among three groups （P〉0.05）. The levels of IL-6, IL-8, TNF-α and MIP-la and MCP-1 were significantly higher in type Ⅱ and type ⅢA groups than those in type ⅢB group （P〈0.05）. The levels of MIP-1α and MCP-1 were significantly higher in type Ⅱ group than those in type Ⅲ A group （P〈0.05）. There were no significant differences in the levels of IL-6, IL-8 and TNF-α between type Ⅱ group and type Ⅲ A group （P〉0.05）. The levels of lgG （（7.06 ± 1.21）, （10.81±1.86）, （15.81±3.06） g/L）, IgA（（2. 37±0. 73）, （3.92±1.04）, （5.89±1.30） g/L）, IgM （（2.48± 0.62）, （3. 57±0.90）, （4.97±1.11） g/L）, IL-6 （（0.28±0.09）, （0.36±0.09）, （0.45±0. 11） μg/L）, IL-8 （（0.32± 0.06）, （0. 39±0. 16）, （0.51±0.18） μg/L）, TNF-α （（0.60±0.09）, （0.67±0.26）, （0.92±0.34） μg/L）, MIP-1α （（149.25±13.89）, （172.66±18.68）, （248.98±21.15） ng/L） and MCP-1 （（241.06±20.83）, （288.61±27.32）, （379.55±32.66） ng/L） in mild, moderate and severe groups were significantly higher than those in control group （P〈 0.05）, higher in moderate and severe groups than those in mild group, and higher in severe group than those in moderate group （P〈0.05）. Conclusion The levels of IgG, IgA, IgM, IL-6, IL-8, TNF-α, MIP-1α and MCP-1 are obviously high, and vary in different pathologic types and degress of CP. The detection of each index is helpful to the pathological classification and the evaluation of the severity of symptoms.
