摘要
目的探讨艾滋病合并乙型肝炎患者HBV-DNA定量检测与血清免疫学指标的关系。方法前瞻性选取青海省中医院2016年1月至2017年12月71例艾滋病合并乙型肝炎患者,对HBeAg阴性和阳性患者中HBV-DNA、HBsAg和ALT进行比较,观察HBV-DNA与HBeAg结果之间的关系;不同HBV-DNA组别(<3 copies/ml组、3~5 copies/ml组、>5 copies/ml组)中HBeAg、HBsAg和ALT比较进行分析;研究HBV-DNA与HBeAg、HBsAg和丙氨酸氨基转移酶(ALT)的相关性。结果 HBeAg阴性组的HBV-DNA、HBsAg和ALT均显著低于阳性组,具有统计学差异(P<0.05),HBV-DNA的阳性率显著高于HBeAg的阳性率(P<0.05)。在不同水平的HBV-DNA组别发现,在>5 copies/ml组和3~5 copies/ml组的HBeAg和ALT均显著高于<3 copies/ml组(P<0.05)。3~5组的HBsAg与<3 copies/ml组的HBsAg无显著差异(P>0.05),>5 copies/ml组HBsAg显著高于<3 copies/ml组(P<0.05)。对HBV-DNA与HBeAg、HBsAg和ALT进行相关性分析,发现HBV-DNA与上述指标均呈正相关。结论 HBV-DNA与血清中的HBeAg、HBsAg和ALT均为正相关,上述3种指标可以作为HBV复制的重要因子,具有较高的诊断价值。
Objective To investigate the relationship between HBV-DNA and serum immunological parameters in AIDS patients with hepatitis B. Methods From January 2016 to December 2017, 71 HIV/AIDS patients with hepatitis B were enrolled in this study. HBV-DNA, HBsAg and ALT in HBeAg-negative and positive patients were compared. HBV-DNA and HBeAg were compared between HBV-DNA and HBeAg. The relationship between HBeAg, HBsAg and ALT in different HBV-DNA groups（〈3 copies/ml group,3~5 copies/ml group, 〉5 copies/ml group） was analyzed to investigate the relationship between HBV-DNA and HBeAg, HBsAg and ALT. Results HBV-DNA, HBsAg and ALT in HBeAg-negative group were significantly lower than those in HBeAg-negative group （ P 〈0.05）, with statistical significance. The positive rate of HBV-DNA was significantly higher than that of HBeAg （ P 〈0.05）. At different levels of HBV-DNA group, HBeAg and ALT in〉5 and 3～5 copies/ml group were significantly higher than those in 〈3 copies/ml group （ P 〈0.05）. HBsAg in group 3～5 copies/ml and HBsAg in group 〈3 copies/ml were not significantly different （ P 〉0.05）. HBsAg in group 〉5 copies/ml was significantly higher than that in group 〈3 copies/ml （ P 〈0.05）. The correlation analysis between HBV-DNA and HBeAg, HBsAg and ALT showed that HBV-DNA was positively correlated with the above indexes. Conclusion HBV-DNA is positively correlated with serum HBeAg, HBsAg and ALT, and HBeAg and HBsAg are important serum markers with high diagnostic value.
