摘要
目的本研究观察定期模拟人和标准化病人训练3个月的临床医学专业学位研究生(临床专硕)学员与正常入科4个月的住院医师规范化培训(规培)学员的临床基本技能考核结果,探讨模拟人与标准化病人训练对"双轨制"临床专硕临床基本技能的提升作用。方法随机入选规培人员60名,设立A组为对照组(n=60),常规临床基本技能培训;随机入选临床专硕60名,设立B组为实验组(n=60),在常规临床基本技能培训基础上,定期进行模拟人操作训练及标准化病人训练,出科时通过阶段考核并收集数据。结果与A组相比,B组临床专硕学员出科考试成绩、心肺复苏操作以及人文关怀、临床技巧方面有明显提升,差异具有统计学意义(P <0.05)。结论在临床医学专业学位研究生教育与住院医师规范化培训即"双轨制"教学衔接中,模拟人和标准化病人相结合模式使临床专硕临床基本技能掌握较好。两者相结合的模式可能是临床专硕为期33个月的临床基本技能质量保障的好方法之一。
Objectives To observe the different results of clinical basic skills assessment between regular simulated patients and standardized patients training for 3 months of clinical professionals students and normal admission 4 months of standardized training of residents students, and explore the promotion of basic skills in clinical professionals of “dual-track system” by this kind of mix training mode. Methods 60 residents students randomly selected were enrolled in this study （group A was set as the control group, n = 60） and were given routine clinical basic skills training. 60 randomly selected clinical professionals were enrolled（group B was set as the experimental group, n = 60） and were given routine clinical basic skills training combined with regular simulated persons operation training and standardized patient training. stage assessment after training were executed and data was collected . Results Compared with group A, there were signifcant statistical diferences in clinical examination results, cardiopulmonary resuscitation, humanistic care and clinical skills in group B （P 〈 0.05）. Conclusion In the connection between graduate medical education and residency standardization training, that is, “dual-track system”teaching, the combination of simulated persons and standardized patients makes the clinical basic skills well. The combination of the two models may be one of the good quality assurance in clinical basic skills training of clinical professionals.
