摘要
目的通过检测健康吸烟者呼出气冷凝液(EBC)和血清中炎症因子,探讨健康吸烟者气道及全身性炎症反应程度,及这种炎症反应与肺功能的相关性。方法纳入30名健康吸烟者和36名非吸烟者作为研究对象。受试者接受肺功能检测,同时测定血和EBC中白三烯B4(LTB4)和8-异前列腺素(8-ISO)浓度。结果吸烟者血和EBC中LTB4和8-ISO虽有增加,但与非吸烟者未见统计学差异(P> 0. 05)。吸烟者EBC中的LTB4与8-ISO存在相关性(r=0. 732,P <0. 001),同样结果显示在非吸烟者(r=0. 723,P <0. 001)。吸烟者EBC中LTB4与第1秒用力呼气容积(FEV1)和用力肺活量(FVC)均存在负相关性(分别为r=-0. 643,P <0. 001; r=-0. 531,P=0. 003); EBC中8-ISO与FEV1和FVC亦存在负相关性(r=-0. 733,P <0. 001; r=-0. 707,P <0. 001);而非吸烟者中未发现这种相关性。吸烟指数与EBC中8-ISO存在相关性(r=0. 463,P=0. 01),且吸烟指数与FEV1和FVC存在负相关性(分别为r=-0. 586,P=0. 001; r=-0. 550,P=0. 002)。结论吸烟者存在气道及全身的炎症反应,气道炎症受吸烟程度影响。吸烟者的肺功能受损与气道炎症存在相关性。
Objective To explore the degree of airway and systemic inflammatory reaction in health smokers and the relationship between inflammatory reaction and lung function by detecting inflammatory factors from the exhaled breath condensate (EBC) and serum in smokers. Methods 30 healthy smokers and 36 non-smokers were ineluded in the study. The subjects underwent pulmonary function test. Meanwhile, the concentration of LTB 4 and 8- ISO in sermn and EBC was detected. Results Compared to non-smokers, smokers had higher EBC and serum LTB4 and 8-ISO, but there was no statistical difference (P 〉 0.05). EBC LTB4 was correlated with EBC 8-ISO both in smokers and non-smokers (r =0. 732, P 〈0. 001 ; r =0. 723, P 〈0. 001, respectively). LTB4 and 8-ISO in EBC of smokers were negatively correlated with FEV1 and FVC ( r = - 0. 643, P 〈 0. 001 ; r = - 0. 531, P = 0. 003, respectively) (r = -0. 733, P 〈0. 001 ; r = -0. 707, P 〈0. 001), but they had no correlation in non-smokers. Smoking index was correlated with 8-ISO in EBC, and negatively correlated with FEV1 and FVC ( r = 0. 463, P = 0. 01 ; r = - 0. 586, P = 0. 001 ; r = - 0. 550, P = 0. 002, respeetively). Conclusion The smokers have the airway and systemie inflammatory response and their airway inflammation is influeneed by smoking degree. Smokers have a certain degree of impairment of lung function, which is associated with airway inflammation. The impairment of lung function in smokers is correlated with airway inflammation.
