脑肿瘤的MRS波谱变化与肿瘤特异性的相关性研究 被引量:5

摘要 目的:探讨多体素氢质子磁共振波谱(1H-MRS)在脑肿瘤鉴别,特别是在单发胶质瘤与转移瘤的应用价值。方法:单发脑胶质瘤患者10例,转移瘤患者7例,均行1H-MRS检查,测量并计算出两种肿瘤实性区、瘤周水肿区各种代谢成分的浓度值。研究指标包括胆碱(Cho)、N-乙酰天门氨酸(NAA)、肌酐(Cr)、乳酸(Lac)以及Cho∕Cr、NAA/Cr比值的变化,进行比较分析。计量数据以(x珋±s)表示,组间比较采用t检验,P <0. 05有统计学意义。结果:所有肿瘤患者的实性区都表现出N-乙酰天门冬氨酸(NAA)与肌酸(Cr)降低,而胆碱(Cho)峰升高。胶质瘤水肿区NAA/Cho比值均低于正常值,而转移瘤周围水肿区NAA/Cho比值正常或略有减低。结论:多体素1H-MRS对单发胶质瘤与转移瘤的鉴别诊断有重要的应用价值。 Objective: To evaluate the value of 1H-MRS in differential cerebral tumor diagnosis,especially on gliomas and solitary metastases. Methods: 10 cases of cerebra gliomas, 7 cases of solitary metastases were collected. All cases were performed MR spectroscopy successfully. The concentration value of various metabolic components was measured and calculated. It is the regions of solid and edema, which have mentioned of two tumors above. Results: All tumor patients showed decreased n-acetyl aspartate (NAA) and creatine (Cr), and increased choline (Cho) peak. The solid area of different tumors showed NAA and Cr reduction, while the Cho peak was elevated. The NAA/Cho ratio in the edema area of glioma was lower than that of the normal value, while, the NAA/Cho ratio in the edema area around the metastatic tumor was normal or slightly decreased. Conclusion: Polysomol 1H-MRS has important application value in the differential diagnosis of single glioma and metastatic tumor.
