血清RBP-4在肥胖型妊娠期糖尿病血清中的水平变化及影响因素分析 被引量:1

摘要 目的探讨血清视黄醇结合蛋白4 (RBP-4)水平在肥胖型妊娠期糖尿病(GDM)患者血清中的变化及其相关影响因素。方法选取2015年1月-2016年12月在该院住院分娩的单胎孕妇118例为研究对象,按照体质指数分为肥胖型GDM组(32例,BMI>28 kg/m^2);单纯型GDM组(30例,BMI 18. 5~23. 9 kg/m^2);单纯妊娠肥胖组(29例,BMI>28 kg/m^2);正常对照组(27例,BMI 18. 5~23. 9 kg/m^2)。采用酶联免疫吸附(ELISA)法检测4组孕妇血清RBP-4水平,对所有受试者血糖、血脂指标进行检测,并对胰岛素抵抗指数(HOMA-IR)进行计算。结果 (1)肥胖型GDM组与其他3组相比,孕妇血清RBP-4水平明显偏高,差异存在统计学意义(P<0. 05),且单纯妊娠肥胖组孕妇血清RBP-4水平显著高于单纯型GDM组及正常对照组(P<0. 05);(2)肥胖型GDM组血清RBP-4水平与孕前BMI、FINS、TG、HOMA-IR呈正相关,(r值分别为0. 562,0. 409,0. 484,0. 618,P<0. 05)。结论肥胖型GDM组、单纯型GDM组及单纯妊娠肥胖组RBP-4水平均显著高于对照组,其中肥胖型GDM的孕妇血清升高明显(均P<0. 05),可见血清RBP-4水平与妊娠期糖尿病、妊娠肥胖均有密切联系,特别是在肥胖型GDM患者中起着非常关键的作用。
