摘要
目的探讨骨科住院手术患者医院感染状况及引起感染的影响因素,为医院感染预防提供参考。方法收集2016年1月-2017年12月医院骨科住院治疗的873例手术患者临床资料,通过医院病历系统对患者病历进行查阅统计,统计相关资料,分析患者医院感染发生情况与引起感染的影响因素。结果 873例骨科住院手术患者中发生医院感染68例,感染率为7.79%,其中呼吸道感染、手术部位感染和导管相关性感染,分别占38.24%、36.76%和16.18%;单因素及Logistic多因素回归分析显示:年龄≥65岁、合并基础疾病、急诊手术、III类切口手术、切口≥2个、手术时间≥2h、住院时间≥15d、有植入物是医院感染发生的影响因素。结论骨科住院手术患者医院感染发生率较高,感染部位以呼吸道、手术部位、导管相关性感染为主,引起医院感染发生的因素较多,应积极干预以降低医院感染的发生。
OBJECTIVE To explore the prevalence of nosocomial infection in the hospitalized surgery patients of orthopedics department and analyze the influencing factors for the infection so as to provide guidance for prevention of the nosocomial infection.METHODS The clinical data were collected from 873 surgery patients who were hospitalized in the orthopedics department from Jan 2016 to Dec 2017,the medical records of the patients were reviewed and statistically analyzed through hospital medical record system,the incidence of nosocomial infection and the influencing factors for the infection were observed.RESULTS Of the 873 hospitalized patients of orthopedics department,68 had nosocomial infection,with the infection rate 7.79%,38.24% of whom had respiratory tract infection,36.76% had surgical site infection,and 16.18% had catheter-related infection.Univariate analysis and multivariate logistic regression analysis indicated that the no less than 65 years of age,complication with underlying diseases,emergency surgery,type III incision surgery,no less than 2 incisions,operation duration no less than 2 hours,length of hospital stay no less than 15 days and implants were the influencing factors for the nosocomial infection.CONCLUSIONThe incidence rate of nosocomial infection in the hospitalized surgery patients of orthopedics department.The patients with respiratory tract infection,surgical site infection and catheter-related infection are dominant among the patients with the nosocomial infection.There are a variety of factors for the nosocomial infection.It is necessary to actively take intervention measures so as to reduce the incidence of nosocomial infection.
