肺保护性通气联合乌司他丁对老年重症肺炎患者呼吸力学、血气分析指标的影响 被引量:11

Effect of Pulmonary Protective Ventilation Combined with Ulinastatin on Respiratory Mechanics and Blood Gas Analysis Indexes in Elderly Patients with Severe Pneumonia

摘要 目的探讨肺保护性通气联合乌司他丁治疗老年重症肺炎机械通气患者的临床疗效。方法 2016年1月—2017年12月收治的60例老年重症肺炎需机械通气患者,随机分为两组,各30例,两组患者均按常规治疗原发病,对照组行传统机械通气治疗,治疗组行肺保护性通气联合乌司他丁治疗,观察两组的患者临床症状及疗效情况。结果治疗后第1天两组呼吸力学、血气分析指标无统计学差异(P> 0. 05),治疗后第7天治疗组均高于对照组,有统计学意义(P <0. 05)。与对照组相比,治疗组机械通气时间、住院时间、VALI发生率及病死率均降低,有统计学意义(P <0. 05)。结论肺保护性通气联合乌司他丁可改善老年重症肺炎患者临床症状,促进患者呼吸及血气恢复,降低死亡率,有效提高临床疗效。 Objective To investigate the clinical efficacy of pulmonary protective ventilation strategy combined with ulinastatin in the treatment of elderly patients with severe pneumonia. Methods From January 2016 to December 2017, 60 patients with severe pneumonia requiring mechanical ventilation were admitted in the hospital. They were randomly divided into two groups, 30 patients in each group. The patients in both groups were treated with conventional treatment for primary disease. The control group was treated with traditional mechanical ventilation and the treatment group was treated with lung protective ventilation combined with ulinastatin. The clinical symptoms and curative effect of the two groups were observed. Results There was no statistics difference in respiratory mechanics and blood gas analysis between the two groups on the first day after treatment ( P 〉0.05). The treatment group was higher than the control group on the seventh day after treatment ( P 〈0.05). Compared with the control group, the mechanical ventilation time, hospitalization time, VALI incidence and mortality were significantly lower than in the treatment group ( P 〈0.05). Conclusion Pulmonary protective ventilation combined with ulinastatin can significantly improve the clinical symptoms of elderly patients with severe pneumonia, promote respiratory and blood gas recovery, reduce mortality, and effectively improve clinical efficacy.
