摘要
目的 研究腹腔镜胆囊切除术治疗高龄胆结石患者的临床疗效.方法 选择大同市第五人民医院2015年1月至2018年2月收治的133例高龄胆结石患者,告知患者本研究的主要方法后进行随机分组,对照组65例患者行常规开腹胆囊切除术治疗,研究组68例患者行腹腔镜胆囊切除手术治疗.观察两组患者手术相关指标,并对患者术后并发症发生情况进行组间对比,评价两种术式的临床应用价值.结果 研究组患者手术切口长度、手术时间、术中出血量均低于对照组,差异有统计学意义（P〈0.05）.研究组患者术后住院时间、胃肠功能恢复时间及肛门排气时间短于对照组,差异有统计学意义（P〈0.05）.研究组治疗费用高于对照组,但生活质量评分（SF-36）优势明显,且两组患者术后并发症发生率比较差异有统计学意义（P〈0.05）.结论 腹腔镜手术治疗高龄胆结石患者具有较高的应用价值,其治疗费用较传统开腹手术高,但可缩短患者的住院时间,对患者的手术创伤更小,能够促进患者快速康复,提高生活质量.
Objective To study the clinical efficacy of laparoscopic cholecystectomy on elderly patients with gallstones .Methods A total of 133 elderly patients with gallstones admitted to the Fifth People＇s Hospital of Datong from January 2015 to February 2018 were included in the study .The patients were informed of the main methods of this study and randomized into groups .Sixty-five patients in control group were treated with conventional open cholecystectomy , and 68 patients in study group underwent laparoscopic cholecystectomy .The operation related indicators of the two groups were observed , and the postoperative complications were compared between the two groups .We evaluated the clinical application value in the two operative methods .Results The length of surgical incisions , operative time , and intraoperative blood loss in the study group were lower than those in control group （ P〈0.05 ） .The postoperative hospital stay , gastrointestinal function recovery time , and anus exhaust time in study group were shorter than those in control group （ P〈0.05 ） .The study group had higher treatment costs than control group , but the quality of life score （ SF-36 ） has obvious advantages , and there was significant difference in the postoperative complication rate between the two groups （ P〈0.05 ） . Conclusions Laparoscopic surgery has high application value for the elderly patients with gallstones . Compared with traditional laparotomy , the cost of treatment is higher , but it can shorten the length of hospital stay and reduce the patient ＇s surgical trauma , and promote the rapid recovery and improve the quality of life .
