双轴点动脉蒂桡神经浅支移位术的解剖学研究

摘要 目的 :为以桡侧副动脉和桡侧返动脉为动脉蒂的双轴点桡神经浅支折叠移位治疗上肢长段神经缺损提供解剖学基础。方法 :通过解剖 2 0侧灌注红色乳胶的新鲜成人上肢标本 ,观测桡神经浅支的血供 ,分别对桡神经浅支、正中神经、尺神经作不同横断面切片 ,观测神经纤维数 ;测量腕部桡神经浅支与尺神经手背支和正中神经间的距离。结果 :①桡侧副动脉终支与桡侧返动脉升支恒定地存在网状血管吻合。桡侧副动脉走行中发 (2 .7± 1.2 )支 ,桡侧返动脉呈扇形发 (3 .7± 1.9)支的滋养动脉分布于桡神经浅支外膜。②桡神经浅支自然分束可达肱骨内、外髁连线 (又称Huelter′s线 )上 (5 .3± 1.2 )cm ,强行分离长度为(2 .5± 1.1)cm ,可切取长度超过 2 4cm。③桡神经浅支平均神经束数与上臀正中神经之比为 1∶3 .14 ;与上臂尺神经之比为 1∶2 .42。④桡神经浅支在腕部至尺神经手背支的距离为 (3 .4± 1.2 )cm ,至正中神经的距离为 (5 .6± 1.3 )cm。结论 :桡神经浅支具有双重血供来源。本术式既可治疗上肢长段神经缺损 ,亦可恢复供区神经支配区的感觉功能。 Objective: To study the anatomic basis of transposition of superficial branch of radial nerve pedicled with two rotation point arteries to repair long defects of nerves of arm.Methods:The blood supply of superficial branch of radial nerve and numbers of fascicles of different levels were observed and the distrance among median nerve and ulnar nerve and superficial branch of radial nerve were measured.Results:①There were consistent anastomosis between the end branch of radial collatorial artery and radial recurrent artery,The outer membrane of superficial branch of radial nerve was supplied by radial collatorial artery with 2.7±1.2 branchs and radial recurrent artery with 3.7±1.9 branches. ② The natural length of superficial nerve of radial nerve was 5.3±1.2 cm above Huelter′s line,disparted length was 2.5±1.1 cm,the cut length was 24 cm.③ The ratio of fascicles between median nerve and superficial branch of radial nerve was 1∶3.14, and between ulnar nerve and superficial branch of radial nerve was 1∶2.42.④The distance from superficial branch of radial nerve to dorsal branch of ulnar nerve and median nerve were 3.4±1.2 cm and 5.6±1.3 cm respectively.Conclusion:There are double blood supplies in superficial branch of radial nerve.This measure can not only treat long nerve defects on upper extremity, but also recover the sensory function of donor site.
