肺功能联合FeNO诊断支气管哮喘的临床研究 被引量:3

摘要 目的探讨肺功能联合呼出一氧化氮(FeNO)在支气管哮喘患者中的临床诊断价值。方法 200例反复发作喘息、慢性咳嗽疑似支气管哮喘患者为研究对象,分别采用肺功能和FeNO进行诊断,以支气管激发和支气管舒张实验及随访3个月后最终诊断为"金标准",判定肺功能、FeNO及两者联合检测在支气管哮喘中诊断灵敏度、特异性。结果 200例患者经肺功能检查并治疗或随访3个月后,最终诊断为支气管哮喘136例,占68.00%。肺功能在支气管哮喘中诊断灵敏度为85.29%,特异性为82.81%。FeNO检测在支气管哮喘患者中诊断灵敏度为75.74%,特异性为67.19%。肺功能联合FeNO在支气管哮喘患者中诊断灵敏度为96.32%,特异性64.06%。结论肺功能与FeNO检测在支气管哮喘患者中均有较高诊断灵敏度和特异性,两者联合检测可提高诊断灵敏度,但可降低诊断特异性,临床中应根据患者诊断需求选择适宜检查方法 ,以提高疾病诊断阳性率。 Objective To explore the clinical diagnostic value of pulmonary function combined withfractional exhaled nitric oxide(FeNO)in bronchial asthma patients.Methods A total of200patients withrecurrent wheezing and chronic cough suspected bronchial asthma as study subjects were diagnosed withpulmonary function and FeNO.Bronchial irritation,bronchial diastolic test and final diagnosis after3months offollow-up were taken as“gold standard”,to determine the diagnostic sensitivity and specificity of pulmonaryfunction,FeNO and their combined detection in bronchial asthma.Results After pulmonary function examinationand treatment or3months of follow-up in200patients,there were136cases finally diagnosed with bronchialasthma,accounting for68.00%.The diagnostic sensitivity of pulmonary function in bronchial asthma was85.29%,and the specificity was82.81%.The diagnostic sensitivity of FeNO in asthma patients was75.74%,and thespecificity was67.19%.The diagnostic sensitivity of pulmonary function and FeNO detection in asthma patientswas96.32%,and the specificity was64.06%.Conclusion Pulmonary function and FeNO detection in bronchialasthma patients have a higher diagnostic sensitivity and specificity,and combined detection can improve thediagnostic sensitivity,but can reduce the diagnostic specificity.The appropriate examination method should bechosen according to the needs of the patients in clinic,to improve the positive rate of disease diagnosis.
